Thank you for helping Ian, you're such a kind and caring person! I was on IV Antibiotics for 2 weeks, when I had Cellulitis in my leg and it was swollen and very red. The IV Antibiotics are much more effective, I am sure the Doctor will know what the best course of treatment would be!

I understand his concern about the surgery at his age.
He really should scoot over to Thailand for the surgery. It may be hard for him to get there but they’ve got some of the best and affordable healthcare in the world and it’s a very short trip.

I like your channel and catch videos here and there. The bottom line is many old foreigner are going to PHL and are borderline disabled or already going to PHL with multiple chronic illnesses.
As an RN my entire career and now working administration for health plans and hospitals. These types of issues are going to only continue to increase.
Unfortunately it’s going to end up being the unassuming poor Filipinos that get stuck being caregivers to disabled patients.
I have done a back surgery.. I would suggest that he try to talk to a physiotherapist that work with ppl post.op and show the x-ray. They can most of the time give advice and talk about rehab.
Ian has a lot of needs, but this is what I would do if he were my dad:
1. Make an appointment with an internist for a baseline physical. He will need an order for blood work and one or two views X-rays of his leg. An EKG or ECG would be good to have now. But he'll definitely need it before any surgery. He should also get a referral to an ortho surgeon and physical therapy. When he eventually sees an Ortho, don't be surprised if the Ortho orders new X-rays to get the views he needs . But ask if he can give general information based on the X-rays Ian submits. But, it's understandable if Ortho requires new X-rays.
-- Have Ian use a wheelchair when young out. Check UA-cam on how to transfer from wheelchair into and out of a car. Do not use a tricycle. The seat is too low. He and the person helping him could get hurt. Ian looks like a big guy.
2. While waiting to be seen by an ortho, Ian should be working with PT to gain strength. Whether he gets surgery or not, PT will help him! PT will need to see the X-rays and any labs he has so they can help him safely.
3. If he can afford it, two 1 - to 2-hour PT sessions a week would be very helpful for his physical and mental health. I suspect Ian has some contractures on his leg that surgery will not be able to fix.
4. Regarding Ian's questions:
A) Pain is normal post surgery. There are meds that can help, but you will have pain.
B) The metal, typically titanium, used on surgery to fix fractures does not need to be removed. Most people (99%+) tolerate the metal well.
I would recommend Ian to drink plenty of water 1 to 1.5 liters per day, despite having to urinate more often. The family can buy a urinal (a plastic container) that he can use bedside. He can either sit on the side of the bed or he can stand to pee. If he can stand, that will be best. Just make sure he has a way to wash his hands well afterward. Most older adults do not drink enough fluid, and it affects their kidneys negatively. Since he's on antibiotics, he needs to protect his kidneys. Of course, this advice is only good if Ian has no fluid limitations. A magnesium supplement would help with stress for his wife and for Ian. Most adults, even young adults (40s), are low on magnesium.

If Ian gets the surgery will he be able to walk without his walker. If the doctors agree he can take the surgery okay, then I would do it if I were him. I feel so bad for him - he's having such a hard time. His wife and daughters are going through a very hard time as well - it's not easy to be caretakers for an ill family member.

Hi Derek (Q)...I have seen on youtube, many people using sad stories to get views and income. I can see your heart is in the right place with your vids.
Ian's family is relying on him for the source of their income. If anything happens to Ian...then no income. At Ian's age, he has many co-morbid diseases such as high blood pressure. Derek (Q)...you would have to ask the doctors what is the percentage survival rate after the operation to see the risk. It is Ian's decision to have the operation or not. My filipino father in law had a stoke and was on maintenance medicine for many years. He also had an ulcer on his foot because of poor circulation and it was hard to promote healing. After the maintenance medicine failed ...I rushed him to a private hospital for a week. He was transfered to a public hospital in Davao City and after another week died of kidney failure. I can assume you have not seen a public hospital yet.

If a consultant thinks you won't benefit from the procedure, or survive an operation he wouldn't do it. You have to have a consultation to see where you stand, and go from there, nobody can advise without a medical assessment and consultation.
Ian does not talk to people well. Im sure his wife takes care of him really well and he needs to wake up and talk and treat her better. He has so many excuses. He has to remember this is the Philippines and if you want help you jave to be patient. I dont understand how this was good news.
