I feel like everyone is misunderstanding that Claire is not making him feel guilty about this. He's making himself feel guilty, his track record is making himself feel bad for it. The only one she was upset about was Laoghaire because he didn't want to tell her about it and she tried to have her burned as a witch. Claire only wants honesty from him and he struggles with that because of his shame not hers.

Yeah...I think the harm from adultery is very much overstated and overemphasized in the USA. If I believed in god and sin, I would not deem it a sin. If two people need each other and do it tenderly and with compassion, it can be a beautiful thing. I think a lot of the teeth-gnashing about it comes from people who are, really, jealous that the adulterers might be enjoying life more than they are. And this habit about decrying the sins of others: it's a petty and mean thing to find fault with others, and dwell on it. In my experience, the most histrionic people crying out for the punishing of sinners are very often guilty of the very same sin themselves. I can think of 3 or 4 TV clergymen who have displayed such infinite hypocrisy.

Nobody expected it of him, but himself. That's why on her first day back Claire keeps hint asking if he has "other ties" and is totally un-shocked to hear he has a son. Her only hope is that he still loves her.
His sister and everyone else keeps trying to toss other women at him after he starts getting over the first grief, but then they think Claire is dead. He believes, hopes, she's still alive so taking up with another woman would be unfaithfulness. It took 18 yrs. before he gave in that Claire was not coming back and he took Leerie, and that was because he wanted the children to be his.
He confessed this because he intended that Claire be clear and absolute that he wouldn't lie to her about it, even if he could get away with it.

I wondered why he even told her about this. It happened 20 years ago, 6 years after she went through the stones to be precise. She was intimate with Frank, so she had to know Jaime at some point would start sleeping with other women. The important question was answered when she first returned. Did he fall in love with another woman after she left.... That's a resounding NO! So what made him pull this tid bit of ancient history out of his pocket, is beyond me. I wouldn't think he would feel guilty for it, especially since he couldn't have know Claire would return to him ever.

He didn't just decide to do it right after she left. He had no reason to believe she would come back so it wouldn't have been considered being unfaithful. Not really sure why this scene was necessary. In the book, sure, but it just took up a few mins of screentime for a confession that really shouldn't have been a confession. Now if it were told in a normal conversation it would've been okay but I kinda feel like this was just for dramatic effect because the way he opened with his statement made it seem like it was going to be something detrimental to their relationship.

Poteva fare a meno di fare questa confessione senza senso dopo più di vent'anni! E poi con mary un'insignificante donna che non si è lasciata scappare l'occasione di andare con un bell'uomo!

The reason this confuses me is because Jamie told Claire that he'd been with other women while she was gone for 20 yrs. Not sure why she or anyone else would be surprised.
