Thank you. I had my total knee replacement about 4/5 years ago. I can't kneel because of the middle of the knee incision. Thank you very much for explaining the lateral curved incision.
I am 84 years old, weigh 220 lbs, and am 6ft,1inch tall. I have had both knees replaced with the incision down the middle of the knee. I can and often kneel on my concrete shop floor, while woodworking , for short periods with little or no discomfort. I am careful to support myself during the kneeling process so as not to plop down on the concrete but a 3 or 4 minute kneeling period is not uncomfortable.
That is absolutely fantastic! We found that about 15-20% of our patients could kneel with the standard, midline incision that was performed previously.
Wow, if I ever need my knees done I'm coming down to you. This makes such total sense. I had a fancy smancy medial meniscus tear done 6-7 years ago and can't kneel on that knee now. Thank you from this Queenslander I'm saving your video for future reference.
I have had both knees done several years ago ,very pleased with the result but definately cannot kneel. Also if you fall as I have twice it is impossible to get up because you are unable to roll over on to your knees.I have spent hours trying to get up as I live alone.I am really glad to hear things have now improved and I hope that will become the norm in all hospitals.
I've recently had a full right knee replaced. The Doc. told me to kneel asap to get used to it. I can and do. However it is the weirdest sensation as I've never gained full "feelings" in that knee. I started by kneeling in bed, thinking if it hurts, it's a soft landing.
5:00 PM central time I just had double knee surgery, September 18 of 2023 My doctors PA said I can get on my knees and do whatever I want as long as I can handle the pain. She told me do not run unless bear is chasing you because you do not want that hard impact on your knees. My therapist today when I got on my knees for the second time, suggested that I not do that no more. I’ve watched your video games a lot of information and thank you very much for your videos. Have a good day.
I had a operation five years ago and waiting 4 of us started talking 3 of us went to Dartmouth and the doctor made 4 the youngest want to replace his knee to play tennis with his son ,he died on the table heartbreaking,it a serious operation with a long recovery
Glad to hear! The lateral skin incision is in my view, and absolute must if possible. If there are other incisions around the knee, then it becomes less beneficial
Was this due to the placement of the incision? I got a total knee replacement with a Mako robot yesterday, and I'm pretty sure that mine goes straight down the middle. Op was at 8am. I was on the way home at 3pm. Modern medicine is crazy.
My surgeon was amazing but he used a midline incision on my right full knee replacement. Now, through time and much practice, I can put some light weight on the scar tissue. After thinking about it I wondered why the incision wasn't to one side of the knee. Now I know that it can be! Eventually, I'll need my left knee done, and when I do, I'll request a surgeon who does the side cut. Thank you for this informative video!
Im glad to help - its a small change that adds to the benefit of kneeling far easier, whether or not that is beneficial for some patients - its up to them to decide!
Oh my, I didn’t know about not kneeling after knee replacement - - when I had one done ten years ago. I fell a couple of years ago and tried to get up by kneeling, the excruciating pain was so severe that I screamed. I would not get the other knee done because of this, yet it too is so painful that I can not kneel at all now! I always wondered why it is so dreadfully painful to kneel after a replacement - - WOW, I’m glad that I clicked onto this site!!!
Yes, I am making good progress on kneeling following Chien-Wen’s replacement of both knees. Like everything it requires consistent attention to the rehab, moving from very padded through soft through to harder surfaces. It’s especially useful to be able to kneel on exercise mats in Pilates class. It has worked for me1
I had 2nd knee replaced in Jan 2020, (1st one 18 months before). because of Covid I was not able to finish my PT or resume water walking and swimming right away. I did some of my suggested therapy at home but not as much as was recommended. 6 months after my surgery I could not kneel, I would hold on to my walker and slide down onto a thick pad and then sit to do my gardening. I have about 200 ft of roses and other perenials around the edge of my yard which need to be cut and weeded in early spring and mid summer. I have kept working at it, I am now 76 and after 3 years i can just kneel down and get back up with no problem. last winter I had to kneel on concrete to clear my snow blower before putting it away, a little uncomfortable, but doable, I am in US,
I had my left knee replaced 3 months ago by Dr. Liew from orthopadeics 360. I am very happy with the results so far and apart from some minor swelling and some minor heat, I have no ongoing issues. I can kneel on my left knee on hard surfaces no problems at all, although I probably use my right knee to support a greater portion of the total weight at the moment.
I have bilateral knee replacement. I wish my orthopedic surgeon knew about this. Because I have my incisions mid knee and kneeling hurts. I used to be a competitive shooter, but I can no longer assume the kneeling position.
I had partial knee replacement done in 2019 and I can kneel on it providing I use knee pads or a cushion but if I forget and kneel without some form of cushioning the pain is like an electric shock. I was a carpenter so kneeling was a given thing especially doing flooring and skirting boards but now I’m semi retired @69 yrs old so kneeling isn’t so vital but I do have lots of trouble getting up out of low chairs especially without arm rests
1) I definitely can’t kneel after bilateral replacements with midline incision 2) for those who can’t my PT helped me figure out a way to do it ( need to know in case you trip and fall) 3) when on the ground, lay on your tummy. Place a bulky rolled up towel of lap blanket so that it’s firm enough not to squish all the way flat. Place that horizontally across your shins ( between your knees and ankles) closer to the knees though not directly on your knees. Doing this gives your knees enough elevation from the ground so you are not directly kneeling on the ground itself. As you go to get up all the weight and pressure is directed to your shins and your knees ride free because they are neither on the ground or the rolled up towel or blanket. That’s why it’s important to have a firm roll or pillow so your knees don’t end on the floor as you get up. Hope that makes sense. It was brilliant and was the first time I could get down and up from the floor in years ! Going to tell my surgeon about this new lateral approach though - brilliant!
For those of use who have had knee replacement/s (2005 on left 2010 on right), is it possible to have the surgery done over so that one can kneel? Am I correct in thinking that would involve removing the original appliance and then have the knee replacement done again using the cutting method described? I am also thinking insurance would not cover the expense. Comments please!
I’ve had both knees done. I can’t use my knees at all. No cleaning the bathtub, getting something that’s fallen under the bed or even getting on the floor for an exercise because I can’t use my knees to get back up.
I had typical vertical incision on both knees. I can kneel on pads. My Dr said the risk is shattering the kneecap 2:31 that is supposedly glued to a plastic piece. I have spoken to several athletic people who kneel on new knees. This is confusing. My main issue is I want to bend knee and so I can see bottom of foot.
Interesting. I've had bil knees done. Kneeling is very uncomfortable. I do it in my reformer pilates cláss, but, the discomfort is always there and on my mind. Would have liked to have this option. You really need your knees, planting, weeding, getting up from floor exercises or falls! No more kneeling in church, I think God understands

Thanks for the infor, but do you have any suggestions for someone who has both knees TKR? I find it hard to keep my balance when I knee. I’m always pitched forward and if there’s nothing for support I fall on my face, lol.
I have had both knees replaced, however, because of bad back & rt side of neck, cannot kneel in pew at church because of narrowness of kneeler & too close to.pewin front of me. Found this very interesting- especially as I’m getting a total knee replacement in 3 weeks. But, if I may offer a suggestion ? The background music was unnecessary and distracted me from your message. (ADHD issues)
Instead of practicing on a bed or couch, try it on a well-stuffed armchair or recliner. That way, your arms can support your weight easily as you gradually let yourself down on your knees. My incisions, by the way, are just to the medial side of the knee, not the lateral.
Medial incisions in general will have larger numb spots to the lateral aspect of the wound. They are still good. We used to use them many years ago until changing to full lateral incisions..
I was told by my surgeon when i had my right knee replaced that i wouldnt be able to kneel or squat or climb ladders anymore. I can kneel on it but it is painful but i cannot squat. I was the poster child for rehabilitation but actually started regressing asfter 10 weeks with no reason why, both my surgeon and the therapy staff were puzzled
I was never told that I couldn't kneel, just found out that it's very painful so I don't. I also didn't realize it was because of the incision. I might try desensitizing, see if that works.
I connect stand on my knee at all. About 12cyears ago. In stead of having relief I have NEVER been without pain and even my foot sole has no feeling. A no no if only Dr was honest about possible problems I would never have had this painful and now taking me into pain EVERY DAY of my life, operation. Some drs just do not care. I could not even get his reports out of him when I realised I have a bigger problem. By the time I got all my records from other sources together this Dr left South Africa. Sad but true.

Yes you can! 1, Kneel on the ground, preferably a hard surface, and wonder at the eclectic mix of pain and numbness. 2, Have your therapist insist on kneeling down, apparently it’s good for you. 3, And my personal favourite, falling over. See item 1. 4, Sex, still being under the misapprehension that you’ve got it. You haven’t. See item 1.
I tried to get my surgeon to do the side incisions, but he would not. I can kneel as long as there is some kind of padding between my knee and the hard surface.
The curved lateral incision (I have a video on that) is something that has made a lot of difference in my patients. But it is not the normal incision to do.
That’s not the question at all , we want to know does kneeling damage the parts or the changes that the operation causes . Of course we can kneel but should we ? Are we risking damage to the artificial items in our knee ? I have one unipolar and one full replacement. The incision is 15 years old on the unipolar and 5 on the full . So yes we can but should we ??!!
A good pair of knee pads will do the trick. I used knee pads for years before the surgery, I tried without after the surgery, but very quickly got my knee pads back out!
@@helenm6543 - I didn’t say it was comfortable, I just said it was possible. I am two years away from my replacement and just left the constant pain stage.
4 years since left knee done. Absolutely NO, I can not kneel. Flesh and metal pinch, while bone and flesh do not pinch. Small sacrifice for a good functional knee. My new knee is my second favorite body part.
you shouldn't be able to really feel the edge of the prosthesis that easily, unless you are extremely thin. Either way, it should not result in any limitation in kneeling - it is the soft tissue on top of the prosthesis that causes the issue.
Often patients find it hard to kneel even before their knee replacement. We hope to enhance that with the curved incision and removing the underlying inflammation through their total knee replacement
@@orthopaedics360 Just making the point that with a little more care and consideration, this could have been done years ago..... Do find the medical profession (in general) today is more like a large cash slot machine.
I have two knee replacements and do farm work everyday. It is somewhat uncomfortable but if you wear knee pads no problem. I wear Duluth trading company fire hose pants with built in knee pads. Awesome
Since I was a kid, we Catholics always joked that Lutheran masses were for Catholics with bad knees. Because Luterans don't kneel during their masses. 😄😉
Absolutely not! At no point can you get on your knees. I've had both mine replaced. It's more than painful to try to get on your knees. Sit or stoop Only.
I don't know about this guy. I had both of my knees done and I can't kneel down. Pain, maybe. The main reason is loss of range of motion and the knee won't bend like before. I just been at my doctor Dr. Sah in Fremont ca. My last Opp was exactly 5 years back. 130 degrees of bending. The one before 125 degrees only. If your knee bends only that much one can't kneel. By the way Dr. Sah thinks that I am doing very well. Never had any kind of pain.

You have to keep the knee bending and unbending, repeatidly straight after the operation (with attached machine if available, otherwise by own efforts,), in order to preserve a full range of movement. If this isn't done, you lose bending ability.

Found this very interesting- especially as I’m getting a total knee replacement in 3 weeks.
But, if I may offer a suggestion ?
The background music was unnecessary and distracted me from your message. (ADHD issues)
Thankyou for your comments! We'll try to continue to improve.
Ah, one of mine does have a large area of numbness laterally, the other does not.
Hopefully things return back to your good rehab! Every knee is different!
I'm wondering why my surgeon didn't put the suture line lateral to the knee. Since she did not will I ever be able to kneel?
Never say never! This little tip just helps our patients kneel better
I was never told that I couldn't kneel, just found out that it's very painful so I don't. I also didn't realize it was because of the incision. I might try desensitizing, see if that works.
How long after the total knee replacement can I commence kneeling, please?
In general, we start trying about 1 month after the surgery
I have both knees replaced. I have a 2 inch thick pad l use to get on my knees! Hurts and my right knee will swell if l get down on it!
Can you replace a replacement knee that's gone wrong
I can kneel on both replacement knees now, without pain. It took a long while before it was possible. The problem is getting up again!
Can you do squats? I need both knees replaced and would love to go back to regular exercise and gardening.
Yes of course. Squats and leg presses are highly recommended for maintaining muscle bulk and bone density post total knee replacement surgery
@@orthopaedics360,I loved the leg press machine at PT!Wish I had one at home.

I can knell on my rt knee, not my left.
Just had my "bad" one done in November. Doing the second one next month, June. Can't kneel on either one, currently.
Is it OK to kneel if you use a knee pad?
Yes, absolutely
I can’t kneel on my knee replacement feels like I’m kneeling on shards of glass
Where is your incision placed?
I can perfectly no issue at all and can raise myself. I am 77 years old and my knee was replaced 10 years ago,
Worst thing I ever did to my body. Had a doctor who treated me as if I was insane . Can’t kneel and always in pain.
Depends if your tie rod breaks.
Can a patient request the surgeon do a curved incision? I am in the US and need bilateral TKR.
It's best to let a surgeon perform the method that they are most comfortable with.
It's just much too painful, even with cushions.
I can and do kneel. It feels weird but it's not painful.
Hi Robert - no, kneeling does not damage the underlying implant - Dr Chien-Wen Liew
My doctor told me that I can't kneel anymore.
What was the reason behind it?
I had bilateral knee replacement years ago and still feels like I am on razor blades when I try and kneel. However, I can do squats without pain.
Not everyone requires kneeling, but we like to push to increase the ability for kneeling in every way we can.
A good pair of knee pads will do the trick. I used knee pads for years before the surgery, I tried without after the surgery, but very quickly got my knee pads back out!
Sorry, doesn't work. I can't even kneel on the bed or pillows or blankets or cushions. Horrible pain, and I have a high tolerance for pain.
I had both knees done and it's very uncomfortable
So sorry to hear that John. It's best to speak to your surgeon if you are still having discomfort.
I tried and there is no way I can kneel
No. It's like I can feel the edge of the device sharply pressing the patellar tendon.
you shouldn't be able to really feel the edge of the prosthesis that easily, unless you are extremely thin. Either way, it should not result in any limitation in kneeling - it is the soft tissue on top of the prosthesis that causes the issue.
ABSOLUTELY NOT.....both knees are replaced and I can not knell at all. My knees wqill no longer get me up from the floor
Work on quad strength.
@@chuckthomas7758 This will always be a good start, but may not improve kneeing.
But you're absolutely right with respect to getting up off the floor.
The question for me is, Can I kneel *without* a knee replacement??
Often patients find it hard to kneel even before their knee replacement. We hope to enhance that with the curved incision and removing the underlying inflammation through their total knee replacement
I can’t!
Be quick with the answer, the met police need to know.
Too late she cried - Pity when two have been done the old way. Would have hope your profession had addressed this many years ago!
I am sure the best options were chosen for your surgery. This is only an option that we choose for our own patients.
@@orthopaedics360 Just making the point that with a little more care and consideration, this could have been done years ago..... Do find the medical profession (in general) today is more like a large cash slot machine.
No way! I dont kneel because of the pain.😢
I can’t even after 8 years
It is very uncomfortable, not painful
It’s the only negative
Only to the queen of dragons
I have two knee replacements and do farm work everyday. It is somewhat uncomfortable but if you wear knee pads no problem. I wear Duluth trading company fire hose pants with built in knee pads. Awesome
No. Hurts like heck.
If you cant, your clients will be disappointed.
I can kneel but
it's such a funny sensation, I don't do it. Not painful, just weird
No.
Since I was a kid, we Catholics always joked that Lutheran masses were for Catholics with bad knees. Because Luterans don't kneel during their masses. 😄😉
Only if your name is Starmer.
From my experience no.
Nope no way
Absolutely not!
At no point can you get on your knees. I've had both mine replaced. It's more than painful to try to get on your knees. Sit or stoop
Only.
So, until about 5 years ago, the unknown surgical incision, resulted in patients unable to kneel! Why didn't your education teach you this?!#...
No you cant
Are you gonna break
Martha's knee or Elon's knee?
They did this, not me.
You get to pay
the consequences, though.
For being dirt.
Only if you supported BLM.
