An alternative to a ‘gloved’ application can be self massage of the lower abdomen, through the empty bladder to the prostate. Gentle pressure can provide a gentle therapeutic release

I suffered from prostatitis several years ago. It took a year to resolve and 4 different urologist where one told me I was not sick enough.... I performed prostrate massage and it helped a great deal to relieve many symptoms.
At least it doesn't sound like you have damage to reproductive system from going to wrong urologist. I'm dealing with heart issues among other things because of having bacteria pushed out in the body. On my own now medically.

I had Prostate Cancer, had prostate removed with the DaVinci. The physical therapist was teaching me how to do exercises to strengthen bladder for after the surgery. I was not ready for the reaction … she was very professional and kept it from being more embarrassing… yeah, it felt that good …
I have low grade prostate cancer and asked my urologist if a prostate massage would cause problems like accelerate the spread. He said no but also cautioned that it shouldn't be a direct poke/push of the prostate, more of a sideways motion.
Wow! It is so refreshing to see such a frank and straight forward discussion of such topics. I really appreciate that!
Thats some real customer service right there. "I wear gloves to make sure bacteria from my hands doesn't get into the rectum." Not "I wear gloves so I don't get your nasty shit on my fingers."
My first time visiting a urologist. He's a big guy, maybe 6'4" and he shows me his big hands and says, "Hi, I'm Doctor Smith. I have big hands and I really love my job!" Completely cracked me up and relaxed me.

Hello and very informative video. Prostate massage is also referred to as The Army Test. I have been told that when receiving a prostate massage, that if you have one hand on your shoulder then that is ok, but if you have two hands on your shoulder then you are in trouble.
That's a good one. I'll keep that in mind. Thank you so much for the advice.
As someone who has dealt with prostatitis off and on for several years now, I would very much welcome a video of your thoughts on the matter.
Thank you for all that you do Doc
Hey man, I had prostatitis for a long time. One thing that helped me (haven’t had it in years) is drinking plenty of water, and cutting back on caffeine.
@@bekabeka71 oh I apologize, I didn’t know I was supposed to know about every medical treatment in the world.
My doctor was very understanding and gentle with me during my prostate exam. He even said that I should not be embarrassed about having an erection during the exam. I told him that I don't have an erection and he said , " I know, but I do".
In my recent prostrate exam everyone in the Doctor's office was masked up. When the Doctor finished I asked him how my prostrate was and he said I'll be right back. A few seconds later the Nurse came running in and said who was that guy? Not sure what to do now.
For some reason, I sort of knew this video would have the best comments....

I had my prostate examined and massaged at the same time. Dr. Knuckles was extremely thorough. There’s no problem that he can’t get to the bottom of.
I had a prostate exam with a doctor new to me. On entry he said "This can be a little uncomfortable". I immediately reassured him "You will get used to it".
I really like the grade 8 1/2 inch bolts used for drawer pulls on your dresser. Very clever! I may try that myself.
After 10+ years of Chronic Prostatitis, I finally found a way to relax my pelvic floor. I would squat and I would wiggle my spine laterally for 30 to 60 seconds twice day. After doing this for 2 years the chronic pain, stiff legs, hangover feeling, and all the other symptoms of chronic prostatitis finally went away.
Andrew, 2 years is a long time. I assume you found benefit at the beginning to continue it for so long. Do you shift your hips laterally in squat position?
Really. I've dealt with this nightmare 30 years. No infection. Did it really work?
Although you referenced a study (from 2006), another one you listed (from 2008) showed that almost 9% of patients reported improved symptoms after using at-home prostatic massage and the conclusion was that more studies should be done regarding at-home massage. Though it would take a long time to conduct, a study (or studies) should be done with men using proactive prostate massage starting in their early 40's to see if it can provide a prolonged onset of symptoms and/or less severe symptoms as they age into their 60's/70's.
Proststitis is really a catch all term…it has been misused for decades as it really does not exist. No one does pre and post massage urine samples looking for bacteria and or white blood cells. Urology text books still list 4 categories but inthe end, treatment is no different. A true prostatitis results in exquisite pain and usually an admission to the hospital. It is often misused to diagnose a man with a UTI when in reality all the guy has is just that..a simple UTI.
So do I recommend prostate massages? Absolutely no
As an expert in both male and female pelvic floor sexual medicine and reconstructive surgery, there is not anatomical benefit. You are not milking out evil bacteria or gnomes or evil spirits!!! The prostate does nothing but provide a substance that we think aids in fertility but that’s it! And high Vit A or Zinc doesn’t provide any therapy to the prostate either.
So does prostate massage help some people with “I ache down there?” Sometimes…it for reasons you may not know
Prostate massage can cause relief of pelvic floor discomfort or congestion by basically stimulating the pudendal nerves which can be somewhat erotic if done correctly-men have very strong pelvic floor contractions with orgasm which incorporates the prostate. But this same massaging also relieves the pelvic floor muscles which relaxes them and takes the tension off and many men find urinating easier.
I hope that helps

It is really fun and refreshing and it has been a traditional practice in Thailand for hundreds of years and it only cost around 15 dollars.
Your video that discussed the impact of antihistamines on the prostate and urine flow was very helpful. I was experiencing poor flow and as a result, concerned about more serious issues even though tests were normal. I limited my daily use of them and my urine stream is returning to normal. THANKS!
Question: I have a nodule in my prostate and been receiving active surveillance every 4 months for a bit over 2 years now. Everything has been fine, no change noted and my PSA has remained steady a bit below 1. What is the chance that a prostate massage for pleasure, deliberately stimulating it, would cause a disruption or change in the nodule for the worse? Like cause it to break open. And thank you for being so open about this topic.
Salutations, Dr. Malik!
Thank you so much for educating so many people about a subject that most people prefer to remain ignorant about until it directly affects them.
I think it was especially helpful for you to point out that that it is important to use lubricants that are specifically formulated for use in areas that they are designed to be used in, and that it is contraindicated to use them in other areas. I think this is often overlooked and not discussed by a large number of patients and other people that particularly are looking into non-medical applications. Most pharmacies and adult resources frequently do not seem to make the distinction between one use and another, and how the negative affects of improper ph levels can negatively affect subjects biome’s.
Thank you for posting these videos!

The first time I had a prostate exam it was part of an overall physical I had to have done for work. I was about 24 years old, and was nervous as hell about it. For some reason, I thought a little humor might be of use, so I told the guy....sometimes I think of the things I've said over the years and just shake my head....he asks if I've ever had this done before. I said no. Then I followed it up with "I'll try not to dispatch any methane during the procedure." Now, to the casual observer, maybe there's a little humor in that. If you're the guy about to perform the exam, probably not so much. He said "This....is an exercise in relaxation."
No it was not. I'm the first to admit that his line was waaaay funnier than mine, but...no, there was no relaxation to be had.
All I got to say is “if he puts those gloves on, he better plan on doing some dishes!”

Prostate massage gave some relief from what I believe was a blockage of some ducts in the prostate. I had bilharzia as a child and a scan picked up some calcification in the prostate. I had difficulty ejaculating a few times, and several bouts of prostatitis. My urologist suspected Chlamidia despite a negative test, and gave a one shot antibiotica, which gave some relief. I would appreciate more info on prostatitis and possibly blockage in the various tubes.

Could you comment on the possible role of fungal infection being the root cause of non bacterial prostatitis looking at studies such as the one published in the Central European Journal of Urology in 2013 by Kotb et al. as well as other studies showing that non bacterial prostatitis contracted by men under 40 is often fungal related and why this isn't addressed more. Also if you could comment on why tests such as Expressed Prostatic Secretions (or others) are not routinely done to find out what type of bacteria or fungal infection is being dealt with so that treatment can be better targeted. Thanks :)

I had prostatitis a few years back myself. Pain, difficulty urinating and 103 temp. Symptoms ended almost immediately with a dose of Advil given to address the temp when I went to urgent care. Urine sample afterward (first time I'd felt I'd been able to completely empty my bladder in days) and physical exam turned up nothing, but I did end up doing a course of antibiotics just in case. From my research I recall there's some percentage of prostatitis cases that are of unknown cause. Would love to hear Rena's thoughts on this.
Guys …. i cured my recurring bouts with prostatitis by cutting out liquid Cows Milk from my diet. Fortunately i can still eat cheese and the occasional yogurt with no effect… a glass of cold milk an cookies or a big bowl of cereal and milk are delicious i know but it’s a small price to pay for not experiencing the frightening pain and misery of prostatitis …. definitely worth a try !!
as a 76 year old male, i find my diet is key and taking anti inflammatory vitamins help also, i know if i am doing things right by the way i go to the washroom, lol, what i have found over the years is that every body reacts differently to the same treatment. i have also found that DIET is key to your health, if you are counting on pharmacy stuff you

Well embarrassingly it works for me. I was having weak urine stream and pain. The urologist prescribed an antibiotic even though my urine didn’t show any signs of infection but I got the prescription anyway and took the pills and the pain went away and my stream improved somewhat. A few months later the pain was back, I’m missing the toilet and soaking the floor and didn’t want another course of antibiotics cause again, nothing was found in the urine. So I got some toys figured out which fit best and went at it. And behold I was peeing like a champ! Not to mention the pain started to go away and according to my wife my “performance” improved (though she won’t help me with the massage). I am an avid cyclist for many years and never had an issue until one day I had an issue. I massage every few months and everything works as it should (no more urine on the floor!). Yeah it helps, I am proof of it. And yes it’s embarrassing to talk about. BUT we just buried my brother-in-law this past May, he died at 53 from prostate cancer and it was an undignified dimise. So get you psa levels checked yearly and lube up every now and then! Hope this helps

Dr. Rena, do you give a reach around while you do it?

Since turning 40, my priority in selecting a doctor as a general practitioner is that they have very skinny index fingers. No matter how good any doctor may be, if he/she has sausage fingers they are IMMEDIATELY removed from consideration.
DREs are relatively 42-46% accurate, also while looking for tiny fingers you should also look for "delicate" as well. A doctor with rough or callus hands might miss details. It is better to have a PSA blood test, less invasive more accurate, and if you already feel some pain down there you should schedule a CT or MRI scan for a better picture of the situation. Hope you stay healthy!
