As an FNP-C for > 24 plus years, thinking about the person as a whole first and foremost is lifestyle. Partnering to come off medications accordingly when able. Start with the source, not just turn for another medication to the copious medications they are already one.

Wonderful interview with a wonderful clinician and professor! I had the privilege of having Dr. Bowers as a professor at Duke as an FNP student 💙😈#BlueDevilFNP

Is there a quick reference for when and how long to hold anticoagulation and when to use Lovenox, around a procedure?

I am not aware of a quick reference. My personal go to the UpToDate as they give anticoagulation specific guidance in relation to pre-operative management.

I have a question about the warfarin titration example. If INR is low from diet, drugs, drinking or missed doses and you decide to add that half tab for two nights, do you go back to their regular 5mg/d dose after INR is normal a few days later?

There are multiple factors that must be consider in titrating warfarin. If the low INR is related to drugs or drinking a goal would be to moderate this modifiable factors as opposed to dosing change.
