Butcher is literally splitting in two. Starlight has literally been replaced with someone else at the end. This is a show about the helplessness and destruction of self-loathing. Only Hughie and A-Train (and maybe Sage?) have managed to turn the page, but obviously at least one of them won't get out alive.
She also loves her job too much (and hates it the same way), and the perks it brings for her sexual life (she loves to dominate, and being fucking Vought CEO is exactly this). And if cause, sage leaving gives her justification for her to stay (again ... Recurring theme for her this season)

Hughie's mom may have gotten compound V from Ashley.
