The dispersive pad needs to have the same charge as the medication since opposites repel and will drive the medication. If the medication has a positive charge you would need to use the positive electrode attached to the dispersive pad. Using dexamethasone, which has a negative charge, would indeed use the negative charge over the electrode as that medication has a negative charge.

Hello, i have an interesting question. Im doing iontophoresis for palmar hyperhidrosis and i started to have problems with tendons and articulations, mainly my shoulders and elbows. Im 42 and i loved to lift weights but nowadays cant go to the gym cause of my problems and this problems started so sudden that it made me think that the use of iontophoresis device have something to do with this. I have a very extreme case of hyperhidrosis and i need to do like 4 sessions of ionto per week and im setting the machine at full power of 30mA at 58v and its now 3 years im using the machine to keep sweat at bay. Can the electricity to do me harm and destroy my articulations?

I would think that if it was related to the electricity the problems with the tendons and articulations would be in your hands, after all that is the closest to where you are applying the electricity. Since it has been three years I would imagine that is more of a change of time. I have noticed the problems with tendons and articulations and shoulders and elbows is often a mechanical problem that starts with the spine and ribs. In order to sort this out if I were you I would try to find a physical therapist who does muscle energy technique and have them work on your entire spine (not just your upper back but sacroiliac joint, low back, thoracic spine, neck, and ribs) to see if this helps with what’s going on with your shoulders and elbows. That way you will not be adding any other chemical agents but will be handling the mechanics. We have a clinic in Virginia Beach and in Northern Virginia near Dulles airport. If you are near either one of those we will be glad to help you ourselves. I also have a friend who does this in Colorado, Hope physical therapy.

I never heard about this muscle energy technique and unfortunately i live in an eastern european country where most physical therapist knows just to handle an ultrasound machine or tecar, laser, etc. I was so strong without pain and problems and suddenly working out in the gym is hard and although my muscles can handle weight i can not lift and workout properly cause the tendons. I guess its age but in my mind i thought this kind of problems comes after 60 not in my early 40. Aging sucks!

So the terminal can only deliver either (+) or (-) current to a drug of known polarity in order to drive the drug through the skin. So Dexamethasone, for instance, a very popular drug to use in iontophoresis is negatively charged so you put the negative (black) electrode on the patch with the drug.
In this case you are choosing the active electrode based on the drug you want to drive in. So sometimes the pushing electrode is positive & sometimes negative. What would one call the receiving electrode, I don’t think ground is the right term regardless of polarity.
