I like how different people get completely different ideas from this song. For me, it always seemed to be about a guy who married his young sweetheart, but eventually came to wonder if she was The One, or if there was a love out there greater than what he felt then. So he wonders if he found her, or if he lost another opportunity at greater love by marrying her. The first verse is about innocent, exciting, young love. The second verse is where he is now, maybe ten or twenty years later. The "big pill" that's "looming" is his question of whether or not this love was true.
