    This can't be here in America no doctor spend more than 5 minute discussing your symptoms. The medical system in America and most other big countries is broken patience are no longer priority. They just care about making money.. Keep the good work DR. Biswas

    Got this when I was 14 and I’m now 19. The dizziness has never stopped. Nausea on and off. I had to be homeschooled and missed out on high school and lost my friends. Doctors always told me it’s NOT vertigo because I’M spinning, not the room, but the Mayo Clinic diagnosed me with this. I do admit I have anxiety but the anxiety came AFTER I got sick (I have a fear of being sick in public so for 2 years I was isolated in my room from this.) I didn’t get this from being anxious. I was a healthy 14 year old with friends and I loved dance, golf, school, and my family. Nothing in my life was wrong. This isn’t a mental condition it’s very much a physical issue in the head or ears and depression and anxiety definitely comes along with it after being sick for so long.

      Hey there i thinked like when i was at crises but only when i believe that was a psicho issue i start to get better! I just haved the same thing and i'm so mutch better now! Please belive this can end... i will make a video telling my experience and how i'm better now i want to help people like us! Because i thinked i was die or to be crazy!     First get very thoroughly investigated by detailed audio vestibulometry tests and if required imaging studies to rule out any organic pathology and sinister issues. If the properly ( remember that tests are not usually done properly and hence for patients coming to our clinic we do the tests ourselves) done tests rule out organic and sinister lesions then other modalities of treatment will have to be initiated. PPPD is a type of FUNCTIONAL DIZZINESS with very definite characteristics that is treated by a combination of different modalities like desensitisation, special types of physical therapy, cognitive behavioral therapy, proper counseling and sometimes if required SSRIs and benzodiazepines. First it has to be DIAGNOSED precisely and accurately and only then corrective measures started.

    Without taking a very detailed history and caring out the battery of vestibular function tests and some neurologic tests and investigations it is not possible to say whether you actually have any organic lesson that is any definite damage to any of the multiple organs that maintain the body's balance. If everything does that there is NO organic lesson in the system then this needs to be treated on the lines of a FUNCTIONAL DIZZINESS. If one or two SSRIs do not suit you it does not mean all such drugs will not suit you. Treatment of FUNCTIONAL DIZZINESS consists of a mix of 1) counseling by a well trained and committed psychologist, 2) proper vestibular physiotherapy by a knowledgeable and efficient neuro- physiotherapist well versed in handling balance disorder patients and 3) some drugs like benzodiazepines e.g., etizolam and SSRIs. These above treatment modalities are enumerated in order of priority. The first is the most important. The anti vertigo drugs like serc / betahistine / Cinnarizine etc are useless and HARMFUL if it is really a functional dizziness. You do not afford to ruin your life like as stated by you.
    Consult a capable doctor at your place who will not shoe you away by prescribing a cocktail of anti vertigo drugs to hide his ignorance. Wish you a SPEEDY RECOVERY

      @Mukesh Patel If you are really having PPPD betahistine CANNOT logically help you in anyway. There are other treatments for PPPD, definitely not betahistine. Please also know that betahistine is NOT an FDA approved drug. If you are getting any pleasure taking betahistine it is upto you to keep doing it, but I will NOT recommend it and rather tell you not to do so.

    Believe me everyone this is just a psychosomatic condition completely managable and curable it totally depends on you just accept this first that you are okay with this if it happens for lifetime and stop making your self being in a particular position which makes you feel better live a normal life don't pay attention to your symptoms talk to your friends and family read or do your work i know it makes you dizzi believe slowly you will be out from this situation and it will never come back i too have this problem and i am much better now and i know slowly i will be 100 % free from this and i will never let it happen again because only your thoughts and fear can make it come back

    I am dealing with this after covid. I currently live in NY and I have been to doctors everywhere and no one has figured it out. They think it’s all in my head or it’s my anxiety. It’s been 5 months and I just have to learn to live with it.

      Go to the physiotherapist, and to a centre specialised in that, there are several in New York, and in two weeks the dizziness can completely disappear with vestibular therapy

      We are in NYC, and my husband had vertigo, and persistent dizziness/lightheadedness.
      Yes, this is real and is very debilitating. We are told to see a neurologist who specializes in Vestibular migraine, vertigo.
      I am thinking it might be PPPD after a case of V neuritis or BVVP returning. Or all of the above.

    I am glad I found this video. I am suffering through this situation and got to know about it after 6 years of suffering. Did everything from MRI to VNG tests but everything came positive. My symptoms get worse at night, I feel a lot for dizziness, pittish feel is there in my stomach, unsteadiness and too much anxiety is there. I feel my eyes heavy most of the times and acidity a lot of times. Can anyone relate?

      I got such symptoms of feeling dizzy, off balance , and falling sensations 8 years back when anxiety hit me. Initially I used to frek since it was very unusual thing I experienced. All my mri and other test are positive. I stopped going to doctors as they only give antidepressants and sleeping pill. I now have recovered partially I mean the symptoms are not persistent bt they are still there.

    I do not know of anybody in Bangalore who is competent in treating these types of patients. Seek opinion of some local neurologist. But if you are prescribed the cocktail of anti vertigo drugs, know that you have wasted your time and money but do not follow the advice.

    Sir is it a neurological or psychological one or mix of both?I used to have a ocular migraine and than stated having dizziness attack ..and ringing in the left ear last attack caused persistent dizziness when I move

      Persistent Perceptive Postural Dizziness (PPPD) is a FUNCTIONAL DIZZINESS which is a NEUROTOLOGICAL disorder with behavioral elements. It is not primarily a psychiatric disorder. A disease is called functional if it arises from a change in the mode of action of an organ and not due to any structural defect and not due to any pyscho-pathological abnormality. Your current symptoms of buzzing sound in one ear with dizziness attacks may be due to many reasons like Meniere's disease, labyrinthitis or even a very sinister condition like an acoustic neuroma. You need to be very thoroughly investigated and a complete audio-vestibular assessment by a NEUROTOLOGIST is essential. PT Audiometry, Glycerol test, BERA, ECochG and of course the vestibular function tests are mandatory; as per findings of these tests you may even need an MRI of the brain with Gad contrast with spl focus on the left inner ear and left CP angle.     Dear sir
    I visited you on 2015. U diagnosed me ppv n gave paxidep n etizolam 3 months. I am now working in oman bangladesh embassy. 9 months ago I contacted u by e mail u suggest me to take sertraline 50 mg. I took it fr 9 months. Now my imbalance is still going on. Should I continue sertraline or not.
    Plz sir advice me

    What is the difference between Phobic postural vertigo and PPPD.
    Are they the same thing?
    Are bpth treatable and controllable?
    Does Medicine, CBT, VRT help this?

      They are two different TYPES of FUNCTIONAL DIZZINESS ; though there are subtle differences between them, Phobic Postural vertigo is sometimes clubbed under PPPD. All types of FUNCTIONAL DIZZINESS are highly treatable and definitely controllable if not totally curable in the literal sense of the word. This is because these disorders are dependent on the personality type of the patient which is a PERMANENT character of an individual. Personalities with a strong trait of neuroticism are the ones more prone to develop such disorders. Correcting, rather manipulating personality traits are the job of very good psychologists which is not always easy to get. Psychotherapy including but not limited to CBT, counselling reassurance , specialised vestibular physiotherapy to increase the patient's balance confidence and a bit of medication helps in a great way with excellent treatment outcome provided the cocktail of these modalities is blended perfectly by an efficient bartender which is the neurotologist. But for all the therapeutic effort to be successful you need a compliant patient too which also not always easy to get, rather more difficult to net than catching an efficient psychologist who is specially adept in handling such patients.

      Doing house work slowly. I have help for cooking and cleaning, because i cannot stand and cook...or even cut vegetables bending down.

    Hey there, i saw you and i think i have a hope to see you ...nearly 15 years back i had very back spinning episode after that i was always like on boat and doctor said it anxiety but i did not take any medicine and continued to live and work with this after 1.5 a year my accute symptoms were gone but even then whenever i get tensed or sometime after eating something and sometime like going to mountains again started a spinning senation but it subsided after 1 to minute's but 2 months ago i started to feel vertigo and dizziness again exactly like i was having 15 years ago doctor sad it anxiety second doctor said its bppv 3rd says it phobic disorder and given me antidepressants which are making life worse so i cant take ssris because of this vertigo and dizziens i am having anxiety attack kind of panic attacks too when i sit and see an object i feel like its moving i cant walk coz of the fear of falling i ve also taken serc tablets it was working a little but not much i am fed up with this life i cant go outside cant enjoy and even can't lay down coz even when i lay down and close my eyes it feels like something is rotating in my head ...can you help and guide please

      Try sulpiride it is dopamine blocker in the brain and anti dizziness if it didnt work it wont harm you you will sleep and you can feel tje effect of it start working in just 1h

    Hello Doctor I read a google patent from a Russian doctor suggesting the use of Buspirone for PPPD. Can this have promise?

      Yes in some pppd patients where there is predominant anxiety and fear buspirone is effective. I do use it off zmd and on. Talk to a local psychiatrist also for a more professional opinion after evaluation. .

      Etizolam has been found to be effective hence buspirone should also be useful but in most studies SSRIs are recommended

    Hello Doctor.
    I had hearing loss in left ear due to earphones.
    Had vertigo after 1 week And followed by another after 1month (motion sickness)
    No vertigo etc for 1.5+ yrs.
    After that only hearing loss in one side.
    Slowly feeling little clogging and then developed tinnitus.
    2020 5 6 vertigo attacks, tinitus worsens during this thing.
    Currently undergoing tinnitus and functional dizziness medication
    (No vertigo from 10months+ only clogging plus tinnitus now)
    PTA mild hearing loss (constant from 2yrs)
    Vemp Normal
    MRI normal
    Impedendence Normal

    I was just diagnosed with this...its horrible..its like being on a boat all year long and you cant get off...I have to take psych meds now ,.but they have side effects worse than the moving sensation.. I have also got vestibular migraines.. Also I hear my pulse in my ear..I have seen 7 specialists and 3 psychatrists...

      Yes I feel like ground below is just moving upside down😢 horrible feeling but I have learnt that to cure it we have to train our brain to be like normal as it was before. Some medical conditions trigger this and brain has learnt a new way as anxiety stimulates our nervous system and our brain is always in fight n flight mode. So first thing is to accept it and not panick as it will again aggravate the symptoms.

      With medication, it definitely IMPROVES but can recur and many patients require much stronger medication. Treatment is NOT just medication, treatment is a combination of medication, psychological counseling and often vestibular physiotherapy esp the virtual reality exercises under the guidance of qualified neuro-physiotherapists who are trained/experienced to handle balance disorder patients. Sometimes depending on the psychic state of the patient the condition can recur when treatment is again required

    Hi sir
    From last 5 month i have a issue of lightheadness or dizzi sansation when i move my neck or head so many time in routine or rotate quickly than i feel lightheadness or dizzy sansation not all spining and unable to focus on a moveable or running or bouncing things.. all test and brain mri reports found normal
    I feel dizziness after having a prolonged eye to eye contact conversation with someone. Too much focus or concentration is making me dizzy.
    could this diagnosable???
    Or curable???

      Depends on whether you have a vestibular problem or a cerebrovascular problem. First, a very thorough clinical examination, followed by CORRECTLY done vestibular function tests including but not limited to VHIT, and then a MRI angiography to specially look for dolichoectasia in the vertebrobasilar system will be required. If all these are perfectly normal, then treatment for desensitization will be required.

    I’ve teetered for almost a year. Epley manoeuvre worked for me. Then the vertigo returned.
    I’d unwittingly rewired the connection between vestibular nerve and midbrain to be always looking for dizziness - the transition from BPPV to PPPV.
    Now working on paying attention to *anything* but the dizzy. Attention on feet. Ankles. Butt cheeks on the seat. Hoping this will rewire communication between vestibular nerve and midbrain 🧠

      Treatment can only be offered after a definitive diagnosis and after ruling out other diseases with similar manifestations. This has to be done by your doctor. A thorough vestibular function test battery, psychological evaluation etc. have to be done very detailed history taken, clinically evaluated and after that the treatment can be worked out

Ent said no issue in ear Psychologist said now its part of your life bare it till death or adjust with it. I cant understand when I get anxious during any situation spining increase otherwise it is normal. When I drive car or ride bike it never come but during day it shows many episodes for some second and it goes could you able to arrange online appointment so we all get proper medicine and treatment.

It get strarted with clastophobia then ear ringing for some second then diziness comes with shock. Clastophobia gone but still diziness and some time ringing sound remain in me. Pls suggest any solution

      Which medicines? I don't think I have at all prescribed any medicines. We don't do so without examining the patient clinically and carrying out the requisite investigations.

    I suffer from positional vertigo. I take betahestamine. It helps for the sick feeling but not the dizziness. When I bend forward I sometimes fall . Please help me get rid of this horrible feeling.

      BETAHISTINE is NOT the treatment for positional vertigo. If you really have positional vertigo it can only be treated by the requisite LIBERATORY MANEUVERS and nothing else. Betahistine is a bogus drug which is not even approved by the FDA in the US. Hence best is to TOTALLY AVOID it.

      @@AnirbanBiswasneurotologist i agree , I have been prescribed this by my ENT and to me it’s more or less a placebo . It doesn’t make any difference . Tanganil is even a bit more helpful .

    I have Menieres Disease.
    Spontaneous, positional and down beating nystagmus.
    PPPD and central vertigo with many drop attacks.
    So to the person who said this is psychosomatic is truly wrong.

      If you are really having DROP ATTACKS and down beating nystagmus which are objective signs the disease us nit totally psychosomatic. Meniere's disease may induce PPPD as a sequel of the disease but that does not mean that the primary disease can be shoved under the carpet as a PSYCHOSOMATIC disorder. The Meniere's disease will have to be properly treated with diuretics and if reqd intratympanic gentamicin. Your doctor will have to take the final decision.

      Controllable DEFINITELY. Usually subsides with treatment that includes three things together viz. Medicines, physical therapy, and psychotherapy. ALL THREE TOGETHER. It may recur later depending on the psychic state of the patient. But first be sure if it is truly PPPD or something else. Often if doctors can't diagnose something, they label it as PPPD.

      Awakened Soul Yes of course. Stress and anxiety can cause PPPD but there is no medical evidence to prove that there is any EEG abnormalities in this disease. In vestibular seizures or epileptic vertigo which is a different disease abnormal EEG is possible not in PPPD

      The reason I'm asking is...I have a 24 7 imbalance feeling. Worse when walking or standing and don't notice it when laying down. I have one 4 week bout of vertigo back in February after 2 months of chronic tension headaches. The vertigo and headaches went away but the lingering imbalance lingers. I would call this pppd however I had abnormal eeg spikes in the right temporal lobe. I understand the right temporal lobe deals with anxiety as well as spatial orientation. So I'm trying to figure out if these spikes are causing my 24 7 symptoms or if there is some other sort of imbalance. A correct diagosis is crucial to my job as I can't work if diagnosed with epilepsy. My 2 in office eegs were negative and the 72 hr one was the abnormal one. Brain mri was negative. I don't have any significant pain just some stress related muscle tension. I also tend to be the OCD type who lots of stress and anxiety especially dealing with this issue for 10 months.

      Yes treatable and most often even curable. Many factors can cause it, one of them being psychological illness

      You need to have your balance system thoroughly evaluated by a good neurotologist, then seek opinion from a neurologist who is a movement disorder specialist and if everything is normal then a psychologist
