That sucks when you have to respond back on a relight. It happens to the best of us. It happened to me twice. They were both before we had TIC's. That's why I say trash and overhaul is so important. As far as? The tasks that was given to us was RIT. Meaning we are the crew that is on standby just in case a firefighter gets trapped or lost. If that happens we are deployed to go find and rescue them.
My guess given it looks like most of the damage is to the top floor’s floor and bottom floor’s ceiling, I would say it started between the two levels. Could be electrical or chimney malfunction.
