Because the text says cervical collar must be used except in patients with NO intoxication, which means cervical collar must be used in patients with intoxication. Correct me if I am wrong. I think you're right. The passage says in order not to use a collar, not only the patient mustn't be intoxicated but also he doesn't have pain, neurological symptoms, injury, etc. The question is somewhat very general and a bad one.
You're right, and furthermore, it's saying that you don't need to place a cervical collar, if the patient meets all of the following criteria. If the patient is intoxicated, it literally breaks that criteria, so a cervical collar MUST be used. The question and explanation was wrong in that case. The question should have read; When treating an Intoxicated patient, ____ must always be used. And the answer would have been a cervical collar.
if you see the question it says must never be used. This means intoxicated then no cervical collar. if no intoxicated than the cervical collar must be used

i thought the rule of thumb is to use "simple language, simple sentence, simple content" ...to communicate with the patients... these tests are not simple...???
