In my case, no one ask for ID card They required my NIN, Birth certificate, Local government certificate of origin and 2 recent passport. This might not be absolute depending on the state you want to do the passport. I wish you all the best.

1. Yes 2. No. 3. After filling out the form for them as a parent, you will tick that they are minors and you are applying as a parent. You should also be able to fill the consent form which I think will replace the guarantor's form. Just search Nigeria passport consent form online and you will see the form.
I am quite confused about the completed application form mentioned on the requirements to take along for capturing, I can not find any form of such on the portal. Although, I filled in all my details when registering. I want to know if it is the same as the payment receipt.

Don't scan the form. It wont be accepted. Instead, he should scan his document and signature for you to use. You can use the app Adobe fill and sign to attach the signature to the form and print out at your end.

Please I want to also clearify this, I am married but I have not done change of name officially, with this will I asked to submit my affidavit of marriage as I want to use my marital name for the passport?
If you want to use your marital name you will have to do a legal change of name, and present the document to them. You will also have to submit an NIN with your marital name.
@@Nyakno93 You have to swear an affidavit then you take that and do a declaration with any national newspaper like PUNCH it should be around 5k. Then you take that newspaper extract alongside the sworn affidavit and a photocopy of your marriage certificate to any NIMC office.

Hey Abraham, it's the same thing. The local government certificate of origin shows the state you are from. So you can use that.

Hey, the statement of Origin is usually issued by the state, your local government precisely. So you will have to travel to get it.

Print out the application form you filled online. That's what you should carry along with your guarantors form and other documents.

Please don't forge signature. He can sign and scan his signature then you can print it on the form. Just write the name on the passport.

From my experience, state of origin is only issued by your state.

I did mine at National Population in Ikeja. I think you can get it done there. And that price is outrageous. Birth cert shouldn't be more than 1k.

You can claim anywhere, either Osun or Imo. You are a daughter of both states. Collect from the most convenient.
