This scene is actually filler. What really happens was that Itachi used Tsukiyomi on Izumi and made a genjutsu showing their lives together all the way until they grew old together. Then in the end of the Tsukiyomi dream, Itachi killed her and she died with a smile.
In manga or Itachi life story novel, Itachi used Tsukuyomi and showed her full life in just few seconds and killed her. In real life she died young but in Tsukuyomi she lived whole life with Itachi and died of old age.
