There is a 200 feet limit that ALL helicopters must follow, this helicopter was nearly double that altitude, beyond that, where they typically have two flight controllers, they had sent one home early leaving the other to take over the whole airport. In all my years of working in the oil field drilling rigs offshore and land. Every investigation into near misses and accidents. There is always two causes. Number one human error, number 2 equipment failure. But we do learn from these investigations and do whatever is needed to improve on how we move forward. We can and do have the best technology to work with. But humans are just that, human. Have you ever daydreamed about night dreams in the middle of the afternoon. You ever get distracted. It only takes a moment. In the oilfield we are regularly drug and alcohol tested. Especially when there's an mishap everyone on site is tested. We live by the target ZERO safety program. All out certificates have to be renewed every 3 years. You fail you go home.
