Your "big box" contains 25 smaller boxes (5 x 5) but mine only shows 16 smaller boxes (4 x 4). Why is that?

What are the black three dots or lines above the ecg strip

They are 3 second intervals. You can count number of of R waves in a 6 second gap then x by 10 for heart rate.
