Diagnostic HysteroLaparoscopy
इसे shortform में DHL कहा जाता जाता है। किसी भी स्त्री को जिसे primary या secondary infertility हो उसके diagnosis में सबसे ज्यादा महत्वपूर्ण ऑपरेशन जो किया जाता है , वह है Diagnostic HysteroLaparoscopy |
यह ऑपरेशन दुर्बिन के द्वारा किया जाता है। पेशेंट के नाभि पर 1cm से छोटा hole किया जाता है इसके अंदर से एक कैमरा डाला जाता है और यह कैमरा जो female reproductive system है उसका assesment करता है। उसे पहले आपके डॉक्टर थोड़े tests भी करवाते है जैसे की पुरुषोंमें semen analysis और महिलाओ में AMH , TSH और prolactin | यह टेस्ट्स अनिवार्य है।

यह टेस्ट्स प्रेगनेंसी क्यों नहीं हो रही है , इसका कारण जानने केलिए किया जाता है। ये ऑपरेशन करने केलिए छोटा सा instrument uterus के अंदर से डाला जाता है जिसे hysteroscope कहा जाता है। और नाभि द्वारा laparacope डाला जाता है। इसमें hysteroscopy से अंदर की cavity के जांच करने के बाद laparascope के माध्यम से uterus , fallopian tube और ovaries का बहार से position, size और structure देखते है। क्योकि इन तीनो का सही position में होना बहोत जरुरी होता है।
इसमें dye test भी किया जाता है उसे पता चलता है की fallopian tube खुले है या नहीं। इस ऑपरेशन में इसमें fallopian tube को अंदर से भी स्टडी करते है जिसे falposcopy |
जब कभी भी Diagnostic HysteroLaparoscopy with falposcopy किया जाता है तो आपके डॉक्टर आपको इसके वीडियो या फोटोज देंगे जो लेना बोहोत जरुरी है।

Thanks for the information, we’ve a doubt though. We are advised for an IVF and need to undergo a prior diagnostic, either hysteroscopy or hysterolaparoscopy. Considering SSG revealed no tubal blockage and no symptoms suggestive of any infection on the outer side of tubes, is it okay to go just for hysteroscopy or shall we still go ahead with hysterolaparoscopy. AMH is marginally lower. Rest of the fertility and blood tests on both of us are normal. Considering reluctance towards invasive treatments owing to fragile nature of wife, we’d prefer hysteroscopy. Please advise.

I had dhl surgery for pcos (ovarian drilling) in oct 2023. After that I successfully conceived a twin pregnancy in april 2024 . But unfortunately my pregnancy lasted till 20 weeks and 3 days only and i lost it on 29 july 2024 due to premature labour pain because of untimely opening of the cervix . Dr is now saying next time when u get pregnant u should get a stitch after 12 weeks of pregnancy. My question is was this situation ( incompetency of cervix ) created due to dhl surgery in the past ?? Plz do reply sir ..🙏🙏🙏
