Very interesting, in our hospital we do need 2 nurses to sign off Heparin, however the pharmacist dose the heparin. They adjust the rate changes and schedule the blood draw. Sometimes we take things for granted right?
I agree with this, various facilities are going with a more streamlined approach to reduce the rate of errors, and this is by adopting in-house protocols to guide the conduct of nurses, doctors and pharmacists.
