    In this lesson we take a look at one of our staples of treatment in the ICU, mechanical ventilation. This invasive mechanical ventilation is a form of respiratory support that we can provide to our patients when other forms of support have failed, or when we need to fully take over control of their breathing.
    I start talking about some of our indications for why would need to use mechanical ventilation for our patients. While not an exhaustive list, this should provide a comprehensive list of things that could lead our patients to this. Next we talk about some basic principals of ventilation, including a quick run down on the circuit and how the flow of air goes.
    Next we talk about the various complications that can come from our patients being intubated and on a “vent.” Again, not exhaustive, but I cover the top 4 major complications, why we see them, as well as what we can do to either prevent or respond to them.
    Finally we round things out and talk about our goals of care for our patients on mechanical ventilation and ultimately what we are hoping to accomplish as well as some tidbits for the process. At the end of the day, remember that our patients are human beings just like ourselves!
    Hopefully this lesson with have invasive mechanical ventilation explained in a way that gives you a good foundation of understanding as we move in to the next couple lessons talking about different vent modes and settings.

    Hello, just wanted to say thanks for putting up this educational content. I'm a 4th year Harvard Med Student going into anesthesia and doing a SICU rotation this month. These vids have been really helpful. Also, the message at the end of the goals of care about treating the person with kindness - that they are human beings just like us at to never forget that. That is priceless, thank you for saying that as it is easy to become complacent when it comes to bedside manner given the stressful/burnt out training that medicine is known for.

    I’m a fresh grads who currently working in ICU. No working experience in ICU and everything down there are completely new to me. Glad that I’ve found ur channel when I’m completely lost! So touched by the last words u gave in this video. “Remember the basics of human kindness. They place their lives on ur hands.” I could vividly remember there’s a day when my patient suddenly turned ill and all the HCP were doing their best for him. Although he didn’t know what we’ve done, but he know we were trying to save him. The day when he finally get better, he suddenly hold my hands, without saying any words, just a simple gesture and emotion from his face, I sense the appreciation. But unfortunately he couldn’t make it till the end. I hope he remember how hard he affected me as a ICU newbies, and hope he’s now well up there.❤️

At times this job can be incredibly difficult, but its those moments that make you realize what an incredibly impact that you can have on peoples lives, even if its one of the last things that they get the experience. I'm sorry that you had to go through that, but I'm also glad that you got to see the amazing impact that you had on this person in their final days, and that means so much in this world! Take care of yourself and glad that you've been helped by these videos in some way!

    Thanks for these useful information. I’m a mechanical engineering student from Indian Institute of Technology. I am planning to make a better design for the ventilator(currently absorbing all information from everywhere😅). I wish to contribute to society with the knowledge I get and going to get.     Love that powerful message at the end! I always try to talk to my intubated and sedated pts, because I know hearing is the last sense to go... and I feel as though if I was in their shoes, I'd like someone to talk to me as well, and to know what is being done to me. Even though at times I feel like I am yelling through my double masks, face shield, and over the loud rumblings of the air filter we have in the rooms. Although to be completely honest, sometimes I'm also too rushed or exhausted and I'd stop talking... But there are definitely many ways of caring for someone with dignity, from the way you touch them, clean them, position them, etc. Thanks again Eddie for reminding us that what we do is an honor that shouldn't be taken lightly!

      Thanks Katelyn. It does help from time to time to stop and put things in to perspective. It's easy to get lost in the tasks and the things we have to do and easy to lose sight of the fact that it is another human being laying there, probably not much different from you are I.
      I 100% agree though that we don't always have to be talking to them. But I feel strongly that it does matter. Same with tone and things. All that I think gets through and makes a difference, along with all of the other things you mentioned. We have many things in our control which can really matter for them, whether we truly realize it or not.

    Thank you for this video. I recently lost my oldest child & only son at 18 in a tragic car accident. Trying to make sense of it all.     I've done 4 things to greatly improve my lung function and overall wellness. My sleep, energy, and life outlook are greatly improved:
    1. I started using the BreatheEasy Lung Exerciser about 5 years ago and still use it,
    2. I stopped mouth breathing unless absolutely necessary, even when exercising,
    3. I did light mouth taping at night in order to ensure nose breathing all night,
    4. I do an simple exercise to reset my CO2 levels for triggering breathing so I don't breathe so much. See Patrick McKeown's work for more about this.

    Is there ever a point where a ventilator will stop working and not be able to keep the lungs breathing and heart circulating blood? Or is there always a weaning process?

      Definitely not always a way off. The vent doesn't help blood circulate though. Only to provide respiratory support for the lungs.

      Yes. A ventilator is one of, and probably the most common form of life support. Essentially any machine or device that is sustaining life in someone and without it would probably mean the patient would die would be classified as this. And so we have a few different things that could and would fall in this category.
