There is a really, seriously really easy extreme demon out there called Evangelion (yes) and people who have beaten it say it should be an insane demon. idk if its easy enough to be 4th place but at leqst in 5th place would fit very ok
Swing copter is painful, but you can actually get consistency in it because there is a click pattern that works well with it, I managed to pass it about three times in a row in normal mode at one point

I beat spectral tentation a few weeks ago, jumping from my hardest level being a medium demon difficulty. Sadly it is now rated as an insane demon, but I still consider it a big achievement as I skipped the hard demon difficulty entirely. ❤

i personally think hypersonic is a pretty great choice for a first extreme demon. its fun, it's not too short but not too long either, and although there are difficulty spikes (terron's part, for example), it's still overall pretty easy

Acu can be an easy extreme too , the drop is very consistent ! And it’s a modern , so it’s readable. The first 20% could be a bit problematic for players that aren’t very familiar with timings doe.

If your a 60 hertz player, cataclysm is not one of the easiest, I would put acu, or thanatophobia as well. Cata is not easy on 60 hertz because ship on 60 hertz is really hard to pull off. If you play on 60 hertz, go for a level that doesn't have straight flying or extremely hard ship parts, like acu for example.
Maybe I'm completely crazy but, for having tested both, bloodbath is less challenging than cataclysm in my opinion ( i had to do less attempts on practice runs for example ) especially because of the first wave part, and the straight files
For those who play on 60hz I'd recommend: Acu, Hypersonic and Incipient. Still being xdemons so diff is high but if you play icdx with 60hz its easily harder than most medium xdemons

Thanatophobia is a really easy extreme demon. The first 50% you can do it 100% of the time, the rest of 50% goes by quick and the last imput is 85%
