    Back in 1996 my doctor told me I had the highest cholesterol of any 28 year old he had ever seen. Decades later I am still active and feel great! If cholesterol was so bad I should be dead by now. Carnivore since 1983 and not dead yet!

      Medical schools don’t really teach about food, they teach how to treat ppl who are sick. It’s unfortunate that they don’t learn more about nutrition but they have to learn hella stuff

    High glucose is from insulin resistance. Insulin resistance is from myocellular lipid accumulation.

      There are manythings that can cause insuline resistance. Including insulin itself. And the trigger can be and usually is high glucose.

      @@pureabsolute4618 that would be if you kept insulin high via diet for decades and you damage and wear out beta cells in the pancreas. But you don't become resistant from the glucose. You become resistant from fatigue of the cells

      No not at all. People should not be eating the wrong food to begin with and no tests, doctors, nurses etc. would be necessary.

      @@Stuart.Branson. What's the "right" food? Even doctors think eating rice, whole grain bread and pasta are okay if eaten in "moderation." But by getting a fasting insulin test you can head off problems early. But testing fasting insulin isn't standard of care b/c they want us to be diabetic! The American Diabetes Association still wants us to eat 3 full meals and 2 snacks daily.

      There are people in the fat acceptance movement who point to their low A1C as a marker proving that they are healthy. They are totally oblivious to the fact that their fasting insulin is through the roof. Eventually the wheels fall off and suddenly to their surprise they are diagnosed with T2DM.

      I agree with you Caroline. If everyone ate healthy (low carb/low sugar), then checking fasting insulin at a young age not necessary. Sad truth is most people eat the SAD early on in life so damage starts early & progresses. The cost of this bloodwork early on far out weighs the overall cost after we are sick with diabetes, heart disease, etc.

    My father has had multiple mini strokes and has heart failure and arteries blocked many near 100%. He’s gotten stents now, but he does not believe me that he needs to stop eating everything and eat meat for a year or two to heal. I send your videos and he just says different diets for different people. Makes me sad. He’s seen my physical and mental changes but doesn’t believe it will help him. I’ll keep trying and maybe one day he will realize. Thanks for all you do!

      @@p1dru2art some who change do it because the condition they were in before was bad, and nothing else was working, so they try carnivore, and their condition improves, and then they realize that what they were eating was causing the inflammation, so then they know that to stay in tiptop shape (or continue getting better), that the carnivore lifestyle is the way for them.

      Berberine should be cycled as it activates ampk...which is good, ..but it blocks MTOR. Dont take it on exercise days and on a recovery day. Even intermittantly activating ampk has been shown to promote longevity in older adults. The benefits of berberine are pleotrophic ( many) including modulation of bad gut microbes, knockdiwn of tmao producing bacteria, liver enhancement and support of the pancreas...among others.

    If you check your blood glucose 2 hrs AFTER one of your major meals it should be < 180. I'm an RN and do diabetes education/management of patients.

      Diabetes can be put in remission. One has to cut carbs and sugar while also (at the same time) find a doctor who is willing to watch your medication and possibly wean you off it.

    My post meal glucose is always so low. Usually, around 95-110, which always makes me wonder if my post meal insulin levels might be high?? Any thoughts?
    I finally talked to my doctor into doing a fasting insulin(Which was normal), but he won't do a post a meal insulin because he said there's no need for it, even though everyone in my family is diabetic and my mother died from severe CVD. He is a vegetarian, so it makes sense. I do take berberine morning and evening.
    Before anyone says get a new doctor, I can appreciate that, however, in my town that's nearly impossible to do these days, so I have to work with what I have.

    Again, the study isn't specific as to what the exact diet was among the participants. There are healthy carbs and unhealthy carbs. There's a big difference between eating a cup of sweet potatoes vs a fastfood burger or processed ham or turkey sub. Also, how many hours of higher level cardio did they do each day, if any? Because I highly doubt they had your physique.

    I am back to eating fruit before my 5k daily walk. Melon, cherries, half an apple, sometimes kiwi. It is hard for me to stay away from fruit in Summer.

    I am 50, do Keto&IF and am regular with Olive, Omega and MCT oils. Eat lower amount of the right carbs and very minimal sugar.
    Luckily am metabolically fit, but with "high" cholesterol that made my doc freak out. Lol.
    Low:Triglycerides, Glucose&Insulin Fasting, C Reactive Protein. But oddly a high Apo-B.
    Did a CT Calcium Score-low dose scan for heart&artery blockages. Zero. Means no deposits to be found 🙌

    I would assume those on low-carb/keto/carnivore based diets would not react well to a sudden increase in carbs, at least at first (assuming they have been low carb for quite a while)? A post fasting test for someone like this, trying to eat some carbs they have not ate in sometime, may result in a pretty significant spike?

      It is the first meal effect. Ben Bikman explains it. Your pancreas needs a little time to ramp up insulin production.

      Fasting and/or low carb diets lower your insulin resistance, so when you do consume carbs or sugar, you don't spike as much because a smaller amount of insulin will get the job done. When your fasting glucose level is running higher than normal because you keep flooding the system with too many carbs too often, then when you spike, you'll be spiking off the higher baseline rate, so you will spike even higher.

      @@kerim.peardon5551 after years of almost no carbs the body forgets what to do with carbs. Takes a day or so to go back to processing carbs. If you ever need to take a glucose load test just eat carbs for a week before.

    At 13:28 in, Table 4 is presented. The R² values are sorted by increasing value, putting LDL at second highest R². That seemed at odds with what you said. But the beta coefficients were mostly sorted high to low. Looked up beta coefficients for multi stepwise correlation:
    Beta coefficients also indicate which variable has the strongest unique contribution to explaining the dependent variable. The ratio of the beta weights shows the relative predictive importance of the independent variables.
    I am a math major with a hate for statistics. Help us understand which column in Table 4 indicates that LDL correlates least to ASCVD. Seems like it is beta.

You did convey the basics correctly in terms of ranking, but you didn't actually provide the correct interpretation of the multiple stepwise regression table. As you read down the table the accumulated amount (over all the previous variables) of variation that is explained is shown. This means that LDL explains .688-.687 = .001 of the total variation. Basically, nothing! LDL doesn't explain any of the variation. Additionally, the coefficient shown (-0.040) implies that for this population a higher LDL was associated with less cardiovascular disease.

    Well, isn’t true that eating vegetables does not raise your post meal glucose level as much?
    So in terms of raising post meal glucose level. Vegetable would be the lowest followed by meat and fat, then carbohydrates. Isn’t this correct?
    So according to your cited study, being vegetarian would not be all that bad. The only problem would be that you would not get enough protein.

      I did said that vegetarian may not get enough protein. I preferred to eat meat myself.
      But there are plenty of vegetarian in the world who live to be centenarian and their post meal profile may be the key reason. We cannot dismiss that according to this study.

    When looking at table 3 and pulling insights on LDL I think you may be interpreting it incorrectly. LDL has one of the highest correlations it just isn’t as important (Beta value) when taking the other variables into consideration?

    Carbs and "vegetable oils" aka highly prosecced seed oils are the real problem. There is different types of LDL-C and the oxydized and small LDLs are the bad LDLs that cause placking.

    I found that even my most intelligent friends have a nearly religious furver about their current beliefs on health, bio markers, LDL, Statins, etc, etc. They have their "priests" who preach the doctrine they've committed to....but will not consider reexamining these topics now and then. Good science can always withstand scrutiny.

    So insulin resistance points to a higher risk of Coronary Artery blockages. This raises the question, what is the cause of insulin resistance? Is there proof that higher carb consumption is the main reason for insulin resistance, or could there be other factors, e.g. seed oils?

    When do I think about heart disease? When I see someone smoking. When I see a big rig diesel truck with soot jetting out his exhaust. When I think about the smog from the 1960's. Also during the holiday season when stores reek of scented products. I study the environmental side of heart disease.

      Funny how Ancel Keys deliberately led people away from cigarettes, given that he worked for big tobacco and included cigarettes in the K -ration he developed. Funny how people just blamed fat in the 1950s and 1960s and not the rampant cigarette smoking and tobacco use. Funny how today, the sickest patients I used to see, were the tobacco users, and more specifically, the old smokers who refused to stop smoking, but also refused to take medications like anti-coagulants to prevent things like blood clots. Oh, well.

    I've long wondered how the human body reacts when a macro nutrient that it has no dietary need for (since it's produced endogenously), makes up the bulk of the diet. An interesting experiment for which data is continuously coming in.

    So it's a good idea to keep those carbs under control And eat fats with your meals so as not to spike your blood sugar? If you are a vegetarian I'm would bet you are getting some nice sugar spikes after your meals. And imo, a macro ratio of 1/3rd of your calories come from protein, same with fats, same with carbs and try to have that ratio each meal. I remember back in the day, a 40/30/30 diet was popular.

      The same? Probably not. Some effect? Probably yes, since the glucose / fructose spikes will glycate tissue when they spike regardless of whether you are in a calorie deficit or not

Depending on muscle mass and glycogen depletion from exercise, I was thinking the body may act as a “metabolic sink” and reduce the unwanted side effects if the sugars are actually being out to use, vs being excessive and staying in the bloodstream

    Why make it so complicated?
    We should focus on GKI. IFF your GKI is greater than 2.0 you need to eat fewer carbs.
    Obviously we all need to never eat seed oils.

      120 grams of carbs per day is my sweet spot, walking 8000 steps per day.
      Resistance training 3-4 times per week.
      2-3grams of proteins per kilo body weight per day.

      @@lokey987 The things I consume have very low carbs in them but when I add everything up e.g. Carrots, Wild strawberries and wild blue berries, Celery, Asparagus, Broccoli, onions, nuts, cocoa, I hit around 100-120g carbs.

    Mike I disagree with you about eating a bunch of carb to replace the standardized amount given on the glucose tolerance test. If we do what u said we won’t really know what amount or carbs we took hence the processing by ur organism might vary from the range in the test giving falsely abnormal or normal results my friend. If that was the case the lab just told you to eat something at home before doing the test and save themselves some bucks.

    I understand it wouldn't give you insulin values..but doesn't wearing a CGM give you those numbers? I wore one for a month and could see what my glucose numbers were after a meal was eaten. Though I wasn't really sure what was awful other than of course what it was after eating something like a milk shake.

      I certainly think wearing a glucose monitor is very useful. It educates you on what happens to your blood sugar in the real day to day world after whatever you really eat on a daily basis. You know the name of the game is to keep the glucose spikes as low as possible, so modify diet and exercise routines to optimize glucose down as much as possible. Then add into that fasting and post prandial insulin tests as he describes.

    There is increasing evidence that endurance runners are at risk of heart disease. Because of their running they are more likely to survive a cardiac event but they have higher than expected heart disease nonetheless. Some doctors and researchers are warning against endurance running. I have been wondering is it the endurance running or is it all of the carbohydrates that they consume to improve endurance.

      Lots of long distance runners like the legendary Jim Fix, have fallen over with heart attacks. Many run 50 miles per wk or more. This just puts too much stress on the heart. You may have noticed that these people do not look healthy.

      @@marktapley7571 I recently read an article written by a physician from about 10 years ago. He had been a runner for about 40-50 years including a couple dozen marathons. He went in for a CAC score confident that he would get a good result. He was surprised that it came at over 1400. People like him are why I ask. Not the "too much strain on the heart" hypothesis.

      @@marktapley7571 I recently read an article written by a physician with about 40-50 years of running experience that included about 2 dozen marathons. With confidence of a positive result, he got his first CAC scan not long before the article. He was surprised to discover that his CAC score was over 1400. He is the reason that I am curious about the effects of carb loading and endurance running on cardiovascular health and not the "endurance running stresses your heart" hypothesis.

    I have coronary artery disease history in my family. Both my older brother and my father had bypass surgery following a heart attack. I was trying to be proactive so I visited the cardiologist while I’m not experiencing any symptoms. He immediately put me on statins, which is raising, my glucose levels. I’m considering taking Metformin to counteract side effects of the statins raising my blood sugar. I’ve read that the type of statin he has me on is not good to be taken with Berberine. Would you have any input on this?

      I'm not a doctor. If I were in your shoes, I'd be doing a coronary artery calcium scan, and maybe a ct angiogram, to actually assess if you have any existing disease. If not, avoid processed foods, sugar, and grains. Not sure I'd be taking lifelong drugs, affecting other contributing factors, without first knowing if there is actually anything to 'fix'. Just the 2cents from a random internet user 😊

      Seems like a horrible choice - developing CVD given your family history or diabetes over time from the statin he has you on. Per my Endocrinologist they are not sure why some statins or even the new cholesterol lowering drugs like the PCSK9 inhibitors cause diabetes in some people vs others, but it's a tough conundrum to deal with. Maybe one of the GLP1 drugs like Ozempic might address both issues?

    It almost sounds like you're saying sugar is bad? The body can only uses glucose and fat as fuel. A better explanation would be why the blood sugar levels are so high. The reason is mitochondrial disfunction. Damged from seed oils and processed foods. When the metabolism is working properly, fruit and honey and maple syrup all organic get metabolized just fine. You saying sugar is the problem is the same thing as saying LDL is the probelm because it gets oxidized isntead of why did it get oxidized? The question is why isnt the mitochondria oxodizing glucose into ATP simply put why isnt Mitochondria turning glucose into energy? Because of seed oils and processed foods. Healthy sugar isnt the problem. Metabolic disfunction is. Going low carb is the same as going low LDL. Its not good. Low carb raises glucagon, cortisol, and adrenaline. All stress hormones signaling scarcity or famine.

    I don't get why people take AMPK driven supplements with a meal, even the last one before a fasting period. If you define a shorter eating window, you have to profit that window to give mTOR a chance, so you can hold on to some muscle, and that includes the digestion of the last meal. If you take it with AMPK boosters, you're mixing signals.
    I believe AMPK boosters should be taken when the energy deficit is kicking in, to amplify and intensify the effects of fasting. That would be at least 2/3h after the last snack/meal (if you're still awake) or next morning...

    Don't use seed oils. They cause oxidative stress throughout your body.
    Then, your stored body fat is oxidized and won't burn normally.
