(I live in kg unit country) There was a questionnaire that people & nurses filled out before they came for free health-check from a volunteer event. I initially thought they filled it out wrong as the BMI wrote 60 Turned out they were correct and I had to ask for help to get the waist circumference so I didn't end up giving him a hug....

He might be the kind of guy to “tough it out” because acidic discomfort from the consequences of his diabetes is somehow more comfortable than getting on a treatment plan that could mean his family gets involved in his health life. I’ve powered through cramps because “whoops, I missed a bc dose. I’ll take the dose next time, but I’m not going to ask for advil right now because I don’t want mom think I’m an idiot for forgetting it the fifth time this month. If she does find out, she’s going to nag me to keep on schedule, which mom doesn’t realize is just annoying and not helping.” In that case, the cramps are indeed mildly painful, but it’s only a bit of discomfort until the next dose I take goes into effect. The discomfort from my mom shoving memorization techniques that aren’t going to help is a much more severe discomfort by comparison.
I don't think that's what the scene is implying. He knows he has diabetes and probably does take his insulin, he just tries to keep it a secret from House/the nurse because he knows that if he reveals he's on insulin, they won't give him the Viagra. That's my reading of the scene anyway, look at his face after House explains his reasoning. looks more annoyed than surprised to me

Hi🎉
Treating Diabetes type 2 with insulin is dangerous. Nowadays we have better methods and better meds.
Nowadays they don't prescribe crap like metformin anymore. In the hospital and up to date doctors prescribe insulin to control blood sugar.
