From what I saw as a ship's officer the answer is simple - cost. Some countries they have to buy their crutches and in others the rule seemed to be for amputations: above the knee crutches, below the knee basic alloy shaft peg leg. Similar in the UK even in the 1930s. My wife, disabled herself, said there were just too many, in one sub-continent children were deliberately disabled so the 'carer' could join the Guild of Beggars .....my father saw this pre-war.
