This is what I came to after 17 years of chronic pain. I realized that I need physical activity (even through pain) to make my body stronger and more resilient. I am now 45 years old. I work out at home. I have found out through experience which exercises make my pain worse and I do not do these exercises. I started with light loads a few months ago. At the moment I work out 30-50 minutes daily. I have a rebounder on which I jump for 20-25 minutes and two kettlebells (5 and 10 kg) which I use to restore strength to my muscles. My chronic pain has not decreased but my body has gained strength and endurance and I like it. Physical training gives a lot of endorphins (especially rebounder) and I have become addicted to them 🤸🏻♀️🏋🏻♀️
Ty dr Ryan. Spondy with a couple bulges patient here. Ty started lightly doing bridges dead bugs planks hopefully working up to get these really weak muscles will support my back.
A few months ago I fell in the kitchen and hurt my back. It was very very sore for a while. I had been walking every day so I just kept on walking up and down the hallway in my appointment. After a while the pain began to lighten up. I have been walking every day ten times up and down the hallway. The pain has reduced and I am getting stronger day by day. I am beginning to feel better and I will keep going everyday. I can also add a lap each month as my back strengthens. It seems-to be working well. Thank you for your good advice.
Wow ! This is the first time I am thinking that I may do what I use to do again. I am scared to do a lot of things after I had herniated disk l4 and S1 about two years ago.I get injured after playing with my son in the park a month ago and now I’m back in the cycle again
Agree to most. However I am surprised: You recommend leg raises -> creates shearing forces. Crunches/SitUps: Flexed LS while being compressed and loaded by long lever. I prefer very slow CurlUps and isometric holds. And I am integrating in all gym exercises (bw/TRX/KB/DB -> closed chain only) making me stiff around lumbar spine by creating intraabdominal pressure (via breathing technique). 100% painfree despite massive L5S1 prolaps caused by bike accident. Age 51. Most important excercises: KB Sumo DL, Sideplank. Was hypermobile before, but then understood joint by joint approach (stiff-flexible-stiff-flexible -> from neck to hip). And Abs are meant to stop motion (twisting lumbar spine) rather than creating motion.
Thank your sharing, and we want to clarify that Dr. does not recommend those exercises. The only thing we see is a b-roll animation of crunches representing someone hurting their back doing exercise, which we admit was poorly placed by the video editor. But there is no point where Dr. Ryan talks about the exercises you mention, and they are also not in the protocol of CBT.
I have a huge incisional hernia from a liver transplant 5 years ago. No surgeon will touch it. It doesn't hurt. I also have difficulty walking and a lot of pain since that surgery. My mri shows herniated disks at L4 and L5 and severe stenosis. I have walked perfectly for 69 years. I am gettng spinal injections every 4 months. they help me walk without pain, but don't last long. I must walk again. Now I am so afraid of falling I hardly leave the house. Please please help me. I have had pt and all they do is put me on the treadmill which really hurts.
What if you suffer from SI joint pain and instability… constant clicking popping etc but you don’t know specifically what caused the breakdown? Also might feel fine when I wake up, but within an hour of normal activity can hardly walk 😢
Hey doc, is this applicable for spondylolisthesis grade 1 as well. I've been working on my core and back strengthening for around 4 months now. Can i gradually start with jogging, gym or any sport?
Going on 3 years now. Tests only show moderate arthritis in lumbar region from L-4, 5 to S-1. Lost 20 pounds, strengthening, stretching, acupuncture, injections, yet am still in chronic pain 24/7/365. I don’t wish this on anyone. It clips your wings and changes your personality- not in a good way. Arggg!! Anyone have ideas?

I believe I’ve torn ligaments that haven’t been picked up by xray!! I need an ultrasound scan. How did you injure your back? I did it doing leg press and have never been the same since!
@mattinfinity1623 Sorry to hear that leg press is a no for herniated discs or for the back in general. Do your research it's the exercise to avoid as it puts a lot of stress on the lower back

Sir L5 S1 listhesis gr2 with pars fracture. PLIF Surgery advised.Dr says it can be worsened by delayed surgery.kindly suggest me.

Leg press and lots of things can break the camels back but a lot of things can lead up to it. Each person is different
