Yeah, the whole point of the 10 vs 90% thing about the meaning of the heart was that Ishigami was part of the 10 percent, probably because like Kaguya he's not very social and listens to his classmates gossip or just talk to them in general. The afterpoint of him saying it was a token of his feelings as in "Token of appreciation", but he worded it just vaguely enough that it sounded more like a confession, especially because of the heart thing
as the narrator said that 90% of shuuchin students are aware that giving heart-shaped stuff during school festival meaning confessing while the other 10% are not aware, in which ishigami turned out to be in that 10% group. This line isnt included in this eps eventho its in the manga. ishigami didnt know about this probably because of his loner status in school(not having friends to gossip about school n romance stuff), like the guy above me said.
