Hi Dr Joe, I am very interested in this surgery but I am wondering will the lower cheeks become droopy after the surgery since there is no more fat to support the skin at that area? And is there any non-invasive way to tighten the skin after the removal of the buccal fat pat & botox to slim the jaw muscle?
Not enough fat is removed to cause the cheeks to droop. This is not fat that necessarily supports any tissue, but rather fat that provides fullness. Conservative removal does not contribute to drooping.
Hi, I finally consulted with an oral surgeon and he probed the area of the gum that has been bothering me. He said it was 'part' of the buccal fat pad that was pushed under the gum when it emerged during a procedure with my periodontist. It's not hernitiated. I'd like to go ahead and have only this section removed (not for cosmetic reasons). He said that he could cut it and push the rest down into my cheek where it belongs, but he could not guarantee that it would work. What could be the consequences of this. I'm waiting 6 weeks to make my decision.
Considering this at 44 but worried it'll age my face as I don't have a lot of facial fat in the first place and my skin isn't quite as springy as it was when I was younger. I keep hearing people say this will make me look aged within a year or two.
Excellent job as usual sir , I like to ask : 1-how you can you sure of equility of left and right 2-can you reimplant fat as a free fat graft finally i have 2 comments: 1- do this procedures using 7 watt diode laser 2-I put a post operative pressure bandage over the cheeck using rubber from dental impression materials Regards I.Kassem, BDS,DMD,FDS RCS
This was pretty fascinating to me. My case is a little different. A buccal fat pad unexpectedly emerged when my periodontist was fixing the gum above an implant. He stuck it back under the gum so that he could resture the area. Since that time, I feel as if I have some kind of accumulated tissue in that area that is intrusive and more than just bothersome. Whatever it is, I'd like to remove it, if possible so my cheek area will feel normal as the other side. I don't need cosmetic surgery, but I need to get rid of the excess tissue. I'm not even sure it is the buccal fat pad but I'm guessing that's what it is because it's precisely where it was pushed underneath the gum. Any thoughts or advice before I see an oral surgeon?
Thanks, If your buccal fat pad herniated (which it sounds like it may have) it can be located and reduced. Should not be a problem for average oral surgeon. Dr. Joe niamtu@niamtu.com
Dr. I under went this surgery about 10 years ago. I was completely satisfied with the results, at first. Now nearly 30 years of age I am wanting to under go another go round to have more of the buccal fat removed. To give me a even more slimming look. Is this possible to actually have a second buccal fat removal?
Thanks, Dr Niamtu, for your useful information. I will pass it on to the oral surgeon that my periodontist is referring me too. Wish I was in your area.
I am interested in this procedure and am willing to pay but I do not want a conservative result or not noticeable result as I’m very insecure about the shape of my face. I would like it to be noticeable. do I have a say if I want just a little extra removed not drastically but more than conservative?

All this time I thought the buccal fat was near the jowls. What can help with the jowls? And what if you still look cheek/they don't take enough out. Can they go and take more out?
Toni Livingston It is more the cheek than the jowls. Liposuction can help some for jowls and of course facelift is the real fix. You an remove more fat after buccal fat reduction but usually not necessary. Dr. Joe
I have another question related to financial responsiblity, if anyone has an idea. I was referred to an oral surgeon to fix a problem (perhaps removal of a buccal fat pad) which was actually caused by procedures done by my periodontist during the year to fix an implant which eventually let to the loss of the implant plus this extra tissue that I feel in my upper gum. Since he felt this surgery was beyond his expertise, he promised me to send me to his oral surgeon if I could not get used to it. He, himself, performed 5 surgeries on the area in an attempt to fix a hole over the implant he put in. The resulting problem was created by him (albeit unintentionally). I got the impression that he would see me through this financially and otherwise. I don't feel I should have to pay for a problem that he created and assumed he'd work something out. I assumed wrong as I just heard back from him that he won't pay but he'd look into his liability insurance to see if there are any provisions for a situation like this. What can I do, if anything?
Hello Doctor, I'm from NJ. I'm planning to do this pretty soon and I would like to make a consult with you. I have a quick question, any email I can reach you?

Is there a way in mordern plastic surgery to reduce cheekbones. A friend of mine has very wide cheekbones and therefore doesn't like the transition from the side of his cheekbones to the side of his forehead as the side of his cheekbones looks wider than the side of his lower forehead. It's somehow very curvey instead of looking more linear and straight. He read thet a cheekbone reduction is possible in South Korea but he doesn't want to travel such a long distance. Is there a a way to adjust the upper forehead region or somehow reduce the cheekbones and the curvey form from the side so that the transition from the side of the cheekbones to the side of the forehead becomes more similar, straight, balanced and linear? His forehead temples are not as wide as his cheekbones

First off, you don't want to hollow a person. Second, it would be very difficult and invasive to try to remove the entire fat pad as it encompasses the temple to the lower jaw. We do just enough to sculpt the cheeks. Thanks for watching Dr. Joe :)
In the case of that patient, he have a huge fat cheeks, that amount of removal won’t make any difference. Why make a patient go thru all that process of surgery to remove that insignificant amount? If a person pay for something like that, maybe is because he want his cheeks gone and have a substantial difference
can you please show an after picture to see the difference?
