Very nice explanation... My only doubt is that in every case, the internal rotation has occured in such a way that the occiput is directed anteriorly... But in malrotation, why the occiput gets directed towards the sacrum?

defect in shape of pelvis is one of the reason

In extreme deflexion, the sinciput touches the pelvic floor first resulting in anterior rotation of the sinciput 1/8th of the circle and putting the occiput in sacral hollow.
