Annual surveys are done for the background activity with whatever sources are there at the time of survey. Suppose Gallium generator or Technitium generator is kept or iodine is kept, just mention in your report that with this much of source was lying at this was the dose rate in mR/h and in dpm.
Mention the area on approved map as well to clarify from where, which dose is coming.
