Do VW t4’s always break?

I had something like this, but I was lucky it was a heath shield that was getting loose.... It started to make very loud noise at certain revs.....

Unfortunately this is definitely a mechanical rattle but I wish it was just a heatshield

Reverse it up a hill/slope. Does the sound get worst/more prevalent?

It’s still the same kind of rattle in any gear, forwards or backwards at the same rpm but not in neutral
