Craniocervical extensor exercise for whiplash injuries

- Hyperflexion injuries are a common cause of muscle strain in relation to vehicle, skiing, and similar accidents. Studies show that, although there are often no structural damage (e.g. bone fracture), the muscles of the neck atrophy in whiplash patients. Furthermore, the studies show that the degree of atrophy correlate with degree of disability.
Thankfully, these muscles may be rehabbed, but beware that it is extremely hard work and requires tremendous motivation and cooperation from the patient. It may take up to six months for the muscles to respond well, depending on the severity of the injury. In extreme circumstances, more.
This exercise will target the longissimus / splenius capitis & cervicis, as well as the semispinalis and cervical multifidii. As always, seek out professional guidance if you are a patient.
If you do not know how to identify the cervical hinge, look at this video FIRST: • Identify and treat the...
Thank you for sharing your knowledge.✌️
Can I do this exercise with rubber band?.
Hi Sir, is there any upright variation of this if you don't have a bed to hang your head off? I have an exercise ball that I can lie prone on but not able to keep neutral spine as the lower body has to be on the ground for support.
Can i do this on the floor? I dont have a bench like you..
Can this be done on yoga mat or a chair?
Kjetil, where should you "feel" this exercise? For me I feel it (muscles burning from use) about midback, at the level of maybe mid/bottom of shoulder blades about where my (hyper)kyphosis is. Is this appropriate or am I doing it incorrect?
Me too
Extensor groups upper back and neck
How much sets and reps to do and if it can be done daily ?
If shoulder is tingling from doing some of these exercises, which muscle do I need to strengthen? Your videos are full of great info.
Depends on several factors. Differentials are most likely C5 radiculopathy, TOS or axillary nerve impingement. You can try strengthening the teres minor for starters
@@MSKNeurology Thank you for that information. I will start strengthening the teres minor and see how it goes.
How many repetitions of this per day?
Can you give your opinion about neck bridging that wrestlers do all the time? Do you think those are safe exercises and if not, what would you recommend doing instead?
Safe without the hinge, unsafe with the hinge
Should you be in a calorie surplus during rehab?
It looks like you are hinging at the top of the movement. Should the neck be "long" throughout the movement or should we end up in a hinge at the top?
Symmetrical extension; not hinging.
I have atrophy on just the right side of my neck. Would this work here too?
I also have atrophy on just the right side for these muscles…which I’m guessing is part of the imbalance and pain I’m feeling
I have a neck injury and whiplash once again this year totaling my car. Had girl land on my neck in college cheer years ago and have a herniated disc at C4 compressing my nerve and now hypertrophy at C5-6 as well. I also have really bad TMD and retruded mandible .. any suggestions of what I can do to help this? I am onto 25 years old and so far nothing has helped.
Should we combine a few of your exercise videos (in my case for whiplash/tinnitus) in rotation throughout the week ie two or three videos every other day or should a person just stick to one exercise only?
Thank you for all the work your do to help others. Your videos are a breath of fresh air!
Depends
@@MSKNeurology fair enough answer. The question was rather broad and each person has their own specific issues so it makes sense.
I know this is an older video but I was wondering if I could get some simple clarification: Would the regular superman pose/exercise not be enough to engage the muscles of the lower cervical spine? Is this the reason for the initial cervical extension?
Anything "regular" typically sucks when it comes to MSK rehab. That said, I am not sure what you imply with regular. Do as you see fit though. This is what I've found to be the most effective.
@@MSKNeurology Thank you so much for replying, Kjetil. I've read all your articles and you seem to be one of the only people I can find online who has looked at these kind of issues so extensively. There is so much attention given to massaging and stretching, but not nearly enough to strengthening these muscles! I was merely referring to performing this exercise without the initial cervical extension as I see recommended all over but I have a lot more confidence in your methods! Thanks again.
Would this be helpful for someone with C1 C2 instability? I aint holding my adjustments to long and it's been over a year
Read my atlas article.
Can injury to neck muscles be a cause of occasional and mild pulsatile tinnitus? Especially when extensive imaging has not found a cause?
Maybe cervical instability
@@pipo4939 neuro didn't think so and wouldn't proceed further with it
In my research looking for answers to my T after a whiplash injury the answer is yes.
What is that rocking intro music? I don’t see a credit.
My friend made it for me
@@MSKNeurology ok l, I’m a musician; so is there an album or website? The guys playing is solid. I’d love to hear more.
This sounds like an exercise that shouldn’t be done by anyone who has neck instability, am I right?,
It should not be done by anyone who has central canal stenosis. I haven't seen your images. you can probably do it, but without reviewing hte case I cannot give specific advice.
@@MSKNeurology thanks for this info. I confirmed with my atlas Chiro that I do not have this problem so can move forward with your exercises. These videos have been a great addition to my regular atlas adjustments with my Chiro on board. Thank you so much!
Kan jag göra denna träning fast jag har disc i nacken .
Kommer an på hvor stor hernieringen er.
