She blew up, her dying from falling out of the helicopter and blowing up is real. That is how Phantom Big Boss got his arm blown off, a lot of the shrapnel in body and in the skull are the bones from her body.
In TPP if you collect extract all the mother base members on the side ops Phantom Big Boss remembers what really happened and realize he was hallucinating about her surviving.
