  • This is a great way to improve the slide and glide of the Pudendal nerve along its pathway, improving nerve function and potentially supporting relief and recovery from pudendal nerve issues such as pudendal neuralgia and entrapment of the pudendal nerve. This will also help to release soft tissues of the pelvis and lower back, freeing up movement in this area and reducing pain.

    I have been really suffering for 8 years on and off with Pudendus Neuralgia. These exercises really help so much! I have been a bit stiff lately but they are easier and easier to do.

    Sorry for the long post.
    I felt a burning pain under the fold of the glute near the rectum that turned sharp and made it very difficult to walk for a few days last October. It's accompanied a whole lot of other issues in my lower body, it was extremely frustrating and I never got a diagnosis, MRI of pelvis, hips, and thighs came back fine.
    I've been doing PT for general lower body issues (there isn't one part of my lower body that hasn't experienced some level of pain for the past year) since March, I'm still not at my full strength and I still get occasional strains, I think I have a lot of imbalances that are gonna take time to fix. I've accepted that i'm gonna have occasional aches and maybe even sprains but today the burning under the glute came back after being gone for months, and this is the symptom that has driven me the most crazy because I couldn't figure out what it was. It seared like a hot chili pepper, palpation around the whole area did not produce any tenderness, I put ice on it for 15 minutes today and it made no difference.
    I looked online to see what nerves run through that area and the it looks like it's the pundendal nerve, I tried your exercise and immediately felt a stretching of some sorts along the inner edge of the glute, when I fished I felt numbness instead of the searing burn that had preceded it. The pain is highly localized under the fold of the glute, with some mild warm feeling on the inner edge of the glute. Based on these symptoms, do you think I may have entrapment of the nerve between between the sacrospinous and sacrotuberous ligaments?
    I read that this is a common cause of nerve entrapment in people who bike. I don't bike but I do sit a lot because of work, I imagine it has a similar mechanism.

      Hey Mario, sorry to hear about your symptoms. Yes, sitting can be a contributor to this kind of issue for sure. It may be helpful to try the pelvic floor rolling exercise on this channel, it can help to release some of the soft tissues around the pelvic floor. The sacrospinous and Sacrotuberous ligament is a common point of influence, you are right there. There are some very specific osteopathic techniques we can use to ease the tension in these ligaments to improve the patency of this nerve pathway. Are you able to see an osteopath near you?

    Anyone trying this exercise please do not do it holding onto the bathroom sink as suggested, you are very likely to pull it off the wall(I sold bathrooms for nearly 20 years). Thanks.

    To be honest this flossing movement helped me and relieved the pressure on my pelvic. I tried almost everything possible to relieve my discomfort and pain.. I will keep doing those movements every day and I hope things improve and heal.

      There are some ways to roll through the pelvic floor on a soft ball but in terms of exercises, the rest involve stabilising the pelvis according to the patient's specific needs. I don't have a video for pelvic floor rolling but I'll get one together and load it on in the next 2 weeks.

    Do alot of people with pudendal also have hip pain? In my case I have painful hips and pain when going 1 or 2 as well as some numbness when sitting.

    Interesting but I've read that when the nerve is entrapped these flossing techniques can potentially stretch the nerve if it can not move freely making the problems worse?

    I sat down every day for a fortnight on a hard plastic chair to do some computer work. I noticed a twinge in the perineum area first, then it developed into pain and I now feel like I'm sitting on a tennis ball when I attempt to sit down.
    I haven't sat down for 6 weeks, can't drive anymore and am popping pills like crazy. Nothing at helps and the pain radiates all around the pelvic area at times. I was treated for prostatitis and UTI without any urine abnormalities etc. This didn't work and I am at now at my wits end. I have had sciatica for years too. ☹

      Hi, it’s now been 15 months and I have had no relief whatsoever during this time. I have no access in the UK to a specialist in this area. I take Gabapentin and Codeine which eases things and helps me get my daily activities done. But as far as any improvement, there is none. I am resigned to suffering this for the rest of my days I think!

    Been having chronic butt rectal pain for months now and been to numerous doctors. No one can figure out what’s wrong. Started PFT and she gave me these exercises. I’ve tried it twice and both times I’m in excruciating level 10 pain all day after. Not sure if I should continue. Literally had to leave work as I couldn’t handle the pain and was crying. She said do it like half way now. And do some breathing to calm my system but that’s not helping either.

      I’m sorry to hear this. Have you got an osteopath in your area you can consult with? Maybe some hands on work can get things started then the exercises will help more.

    Please do another nerve flossing video for the pudendal neuralgia! I have found nerve flosses to be very helpful, it helped me quickly with sciatica. Unfortunately i cant feel this nerve glide that much. Maybe you have another variation?

      BW B, thanks for your feedback, this is the most effective one for my patients, it may be a movement restriction you have that is reducing its effect. Also, consider that it may still be beneficial even without you feeling it. If its not flaring things, may be worth trying it for a period of time then reassessing symptoms. Best wishes.

      Thanks for your input. I’ve been in pain for 10 months now. I can’t overextended kicking hard. chiropractic doesn’t help at all. Sciatica. This is the cure, I feel it. I know it! I’m excited. I was just thinking this morning of tying me legs to me chest to sleep to try and fix it. This came up in my feed.

    I have been sufferingn his condition the last couple of months. I think mine is more irritation as l have osteoarthritis in my hip aswell which seems to be getting steadily worse. Pain in the groin and genital area.

    I developed pudendal neuralgia as a result of a broken spine after I fell down on the ground from the second floor. I was operated successfully but the bones are still recovering. However, the electric shock like pain in my private parts is killing me. I want to try your exercise, I want to get rid of this electric jolts. Do you think I can do this exercise anytime soon?

    Am I correct here: when you move into hip extension you're also anteriorly rotating your pelvis past neutral? And when you flex into the sit it stays in neutral?

    I m having tingling and nerve pain in inside of left thigh to foot....!!! Also continuous vibration in leg ,,groin and genital area...!!! Told to orthopaedic doctor,,he just prescibed medicines,,,,but no permanent solution..!!!also have pain deep in left lower stomach,,, I think it is nerve trapping in lower back !!! May be femoral or pudendal nerve causing it..!! What is ur opinion according to symptoms???

      Thanks for your comment. I would suggest you get an Osteopath or other practitioner to assess you. You may have a nerve issue, it could also be abdominal/pelvic organ restriction plus many more possibilities.

      Yes, most likely, as the purpose of the exercise is to improve the slide and glide of the nerve over its pathway, however there are many causes for these issues and flossing may not address all of these. This is where accurate diagnosis and treatment can be required to provide more effective management and more complete recovery.

      Hi Tobbe, if you listen to your body, take it slowly and back off when needed then over stretching is unlikely. Your body will give you signals and will protect the nerve stretching to an unsafe length. The objective is this exercise is to improve the glide of the nerve, visualize the nerve (like a rope) sliding through a tunnel and work with the body.

      Great question Ben. Really individual answer there. You could do 1-3 times per day, 3 sets of 4-6 reps with a break or some other exercise or stretch in between. As long as you feel it’s not too much then you could do more.

      Hey BW it really depends on the individual, people get relief either way. Maybe try both and see. Take it gently though if you do it when you are in pain and monitor for changes - good or not so good.

    Hi, Dr. Harvey! I've a verified diagnosis of "Pudendus Neuroathy". I've really tried everything over the years. Can't sit anymore. Used every drug. Should I do this exercise even with a nerve entrapment? And if not, any sugestions?

      Hi there, thanks for your question. First, in my experience, Pudendal neuropathy and entrapment are not necessary the same thing. It’s difficult to give individualized advice without an assessment but you could try this exercise by starting slow, gradual, lower reps and then if it’s feeling ok work yourself up from there. Monitor your progress and listen to your body. There is also a pelvic floor rolling exercise on my channel I find really helpful for patients I see.

      @Jéssica Ferrari Thanks for askin'. Actually not. Just left hospital this morning, where I received a pudendal block. If it WOULD have worked, the next step would have been a botox injection. But as it happens ... it didn't work. No relief after. So the search goes on. Greetings from Cologne!

This exercise is non invasive and I have not seen any issues with it from my community, however I can't guarantee safety or suitability without assessment. The other one to look at is the pelvic floor rolling on this channel. I hope this can provide some support for him.

      Hey k6, it depends on the person and their body. As you can see in some of these comments some people feel alot, some feel nothing with the exercise but feel change later or over time. Not feeling something changing at the time is not necessarily an indication that it hasn't, isn't or wont worked for you.

      Hey Umut, no I haven’t had PNB. I haven’t experienced many of the conditions that I see patients for, however I don’t believe this is a barrier to my success as an osteopath. I don’t consider this a cure and her never made this claim. This video is an offering I’ve put out there that may help release the nerve along it’s pathway…As mentioned in the description, these are published for patients I have seen so they can view and perform these exercises as prescribed specific for their individual presentation. It has been incredible to read how much benefit this exercise has been able to provide for people.

It's been 11 months, so only me and God knows what I have suffered from this disease. my only sin was trying to climb a hill for an hour with that damn bike saddle. And now I'm looking for a treatment video on youtube with hope, like a kangaroo with a sneaky viper in its ass. Sorry if I hurt you with my previous question.

      Hi Jonas, many people have found this effective, for many different issues with the pudendal nerve or other body areas and structures such as pelvic pain, back pain etc. If there is irritation or you feel its not right for you then cease the exercise.

      They can be related Paul, yes. Feeling like you’re sitting on the bone sounds to me like less padding for the nerve. This kind of thing will put you in a more likely category to have issues with irritation on this nerve

    Dr. Harvey - I discovered this video today and tried 2 sets of 5 for my PN pain. It seemed to cause an intense flare-up immediately after. Is that normal and means it is working? I'm willing to get a few flares for some future relief, but also don't want to be doing further damage. Thank you for any insight!

      Hi Amy, thanks for your question, I'm sorry to hear about your flare up. It may mean its working, but you may also have an issue higher up, try pelvic floor rolling to catch some other key structures and redo the floss and let me know how you go.

    Hi Jess and thank you for this video! One question: is it ok to wiggle my rear a bit at the bottom of this stretch? I can feel some tightness being relieved when I do that but I'm not sure if it's counter-productive.

      Hey TJ, thanks for your question. I would say that was just fine if it feels ok. It's so important to listen to our bodies...when we really check in it can be profound, the insights we can gain. I provide these videos to give a generic movement and then with my patients I'm able to give them specifics as to more here or less there, so without that (and even with that) it's about self discovery of where you can take it. If it feels good, it probably is going to help.

    Numb top of penis Medial aspect of thighs both. Erections but not stronger. Pain at circumcised ring during coitus. Ejaculations from prostate. Please help and it is all due to bad sitting posture like happens in cycling

      Absolutely Ahmed, bike saddle is a classic feature of this kind of presentation. Sorry to hear of your troubles. Keep working on this and releasing pelvic floor - you could try the pelvic floor rolling exercise on this channel, and breathing exercises to restore diaphragm and pelvic floor function.

    hi doctor. i am having problem with PN. i don't have any pain but i feel like something is stopping my blood flow to my hands and legs and also to my penis and testicles. there are few nerves which are prominent on my penis and it is shirinked due to lack of blood flow. and having ed and numbness. when i wake up from sleep i feel like alot of fluid is accumulated in my stomach and i feel kind of anxiety when i wake up. but it goes away as soon as i wake up. it happened due to excessive sitting habits. and i fell tightness in pelvic area as well. . can you please tell me are these PN symptoms? and may pelvic floor dysfunction?

      Legend 2021 this sounds like an issue that is affecting more than the potential of the pudendal nerve. Please find a physician to get checked out.

    Started doing your exercise 2 days ago and I was 30% better, now I feel 99% better. I've been doing it daily with my other daily exercises for pelvic floor dysfunction. Why is there not any information out there on the internet about pudendal nerve floss? It may be the cure for me and many other people. Thanks a lot!

      Davide, thanks for your comments, so pleased you have found it helpful. The lack of effective exercises out there has surprised me. I plan to put together more information on this in 2022, as its relatively common issue however from what I'm learning, its not acknowledged enough in manual therapy.

    Hi Dr. Harvey. I have pudendal neuralgia that was diagnosed through a nerve block. I believe my PN was caused by heavy training on my road bike clamped to my trainer. Unlike riding outdoors, I went "numb" quite often when my bike was camped to the trainer.
    I've tried many stretches over the last 6 months, (obturator, various glute, sacrotuberous) none of which provide any signs of relief to my PN pain. I just performed one set of 10 of your pudendal nerve floss and whoa, my PN pain flared big time. Could this be a sign that the nerve flossing can be effective? Should I continue this flossing?
    Thanks for posting the video, I'm now following your channel, there is very little on the internet on pudendal nerve flossing. I never heard / tried it before. I'm now following your channel. Cheers!

      Hey Larry, thanks for your message and the follow. Sounds like you are spot on there, it may be that there is such a restriction in glide that its flaring you up with the exercise, may need to look further up the chain into sacrotuberous and sacrospinous ligament. Have you tried the pelvic floor rolling, this could catch a little of the sacrotuberous ligament? And I would recommend looking at the sit to stand. This may help with your pelvic floor and glutes functioning as you move in and out of flexion. Id be interested to hear how you go. Thanks again, Jess

      @@drjessharveyregosteo.head2313 Hi Dr. Harvey, I haven't tried "pelvic floor rolling" if that is rolling on a foam roller / cylinder. I've only been using a hard rubber lacrosse ball for the last 2 months in which I lay /sit on it to press into various areas of glute musculature and directly on my sacrotuberous ligament to stretch them out. My pudendal nerve flared in the first week of dong that but it has not flared until I performed your floss technique. BTW: I've continued to do the flossing and incorporated it into a puppy pose and then moving into a into a cobra pose and back. That flares me as well. I will try the sit to stand; I've been off of my bike for 10 months now, it'll be nice to activate my quads again.
      I plan to buy a foam roller and try pelvic floor rolling. There are various diameter and firmness rollers available on Amazon, a 6" x 18" wide roller in ultra firm foam seem right.
      Thank you for your response, from all of us PN sufferers. I hope other may find your video, try your flossing technique and find relief from this horrible life altering condition.

      @@velofellow Hey Larry, I have a pelvic floor rolling video on my channel, give that a go but use a softer, air filled ball if you can get one so the rolling is more gentle and doesn't stir up the nervous system. Try to get a softer kind of roller or take some pressure off the nervous system so you can relax the tissues into the roller and get a good release out of it rather than smashing it with heavy pressure. Keep working at it. Can you get to a therapist that does Visceral Manipulation? You could check out the Barral Institute website for practitioner trained in that area.

    I have constant spasms around the right side of my anus and when more severe the right side of my perineum. These spasms cause pain. Sometimes the spasms are not palapateable. When more severe these can be palpated in my perineum and also visually seen around my anus as it puckers and releases. There is also some hypersensitivity around my anus and a feeling as though I need a bowel movement. The worse the spasms, the worse the pain and other symptoms.

They "buzzing" spasms begin usually in my sit bone and then travel up through my perineum and into my penis (usually the head of the penis is where it is the most painful). Has your condition improved? The spasms are insane.. there is no way to describe them as they are like something that I've never experienced before. I can't sleep due to the spasms. I'm just starting down this healing journey. How have you been?

I agree with Anna, The Barral Institute is where I would go to find a practitioner, I have done training through there and the techniques are very effective. You could also try Osteopaths in your area...especially those trained in the Barral work.

    I’ve been doing this some for my PN, I realized I’ve been doing more of a squat after watching this and maybe that’s why it flares things lol but I usually have sharp pain in the clitoral area due to this nerve so I hope this can help me live a normal life again

      There are many places the Pudendal nerve can become compromised along its pathway. An MRI can be used, however I usually just use clinical presentation in conjunction with movement testing and palpation.

      Guys in my case i have peyronies disease scarring in the penis this is causing thr penis pain, hope u dont have that. Why its causing leg pain too no clue if there is a connction, hard to believe , but it seems like. Maybe the same nerves im the spinal cord connected. Sucks.

    I have (what appears to be) undiagnosable lower abdomen and perineum pain. My doctor has recently starting talking about PFT as a potential treatment. I have tried this exercise but I don't feel a stretch anywhere. I think I am doing it correctly. Where/what would I be feeling during this exercise? Thanks

      Hey Luke, some people will feel a release at the time of the exercise, others wont. Some will feel a change straight away, for others its a compounding effect with repetition over time. When I do it, I feel a softening in the pelvic floor area, some people feel a glide movement. If it isn't flaring anything then it may be worth persisting even if you aren't feeling anything directly. If comfortable, you could try to emphasise the end of each movement by trying to increase your range. You could also slow the movement down. I hope that can move the needle for you.

      Good question, it’s difficult to advise with knowing your situation. Sometimes starting with less reps can ease you into it or making the movements smalls then increasing over time.

      Hey there, sorry to hear you have had a flare up. It's tough to say unfortunately, as I dont know your case. It could be a matter of doing less, slower and gradually easing into it. Are you doing any other work on top of this? You may need another step before this to have it support you more.

    Nerve flossing has helped me greatly with sciatica previously. After a week of doing flossing my sciatica was gone. It returned 6 months later, did some flossing again, gone again.
    Hope it will help with pudendal neuralgia too. However, im this exercise i cant feel any tingling in the area, like i used to doing leg nerve flossing with sciatica. Normal?

      For sure, keep going, just go as far as comfortable for you. If you can even just raise your heels then bring that in too. Definitely still worth performing even if not to full range as demonstrated here.

      Hey Shane, the body does amazing things to take pressure off key areas and tissues. The pudendal nerve does not supply the glute or TFL, however, if there is a tension on the pudendal nerve (or any other nerve), joint, muscle, tendon etc. then the body, in it's wisdom, may compensate for this to take the pressure off the area. So, issues with the pudendal nerve can be the root cause of glute or TFL tension, among other issues. A good, thorough assessment is one way to gain more insight on this.

    I just had vaginal scar tissue removed that formed after another surgery. Now i am feeling a heaviness when I walk along with tremors and shaking. I think my pudental nerve may have been affected by the procedure.

      You could try the lower back reach and the piriformis & hip internal rotators stretch. The latter is less specific for pudendal nerve but could support.
