    Very frustrating for those of us(70 yrs) that were told not to take hormones and now have low bone density etc. and keep hearing that we should have.

      Honestly, I wouldn't put everything down to HRT. My mother took HRT for 7 years, she felt great on it, but she has difficulty walking and is due a knee replacement surgery in January. She'll be 80 this year. Meanwhile my aunt who's only a bit younger, never took HRT, only herbal supplements, and she walks 10 thousand steps a day.

    The topic of HRT is not simplistic. What about the very significant increase in quality of life that HRT offers many women--I mean a night and day difference. Also, I'd be interested in the absolute risk stats.

    I was hoping you would get into the details of the Danish study. What kind of estradiol and progestin. Bio-Identical or synthetic? Your messaging is cloudy because this study showed increase risk of dementia with long term use of HRT yet you are glossing over it. What are you theories as to why HRT might affect the brain in a negative way? What are anyone's theories as to why this could happen?

      I’ve read the study, and it said they used estradiol and progestin. Progestin is synthetic and progesterone is made from plants. I found the study disingenuous because they used synthetic progestin and not bioidentical progesterone. Both are completely different from one another.

      from what I've read the studies referred to women using oral HRT not transdermal or vaginal. the more modern studies using estrogen HRT decreases dementia. Which makes sense to me.

      this study is about as relevant as the useless WHi study which used horse estrogen.

    Thank you for addressing this recent HRT study. It worried me so much that I have an appointment with my doctor to discuss going off of HRT. I am experiencing noticeable cognitive decline. I am unsure why, given that dementia does not run in my family, and I have made it a lifelong priority to exercise my mind. I have been on HRT for 2 years, and before that I was on birth control (since age 18) for severe dysmenorrhea. My primary reason for being on HRT is because I read that it was neuro protective. Reading a study that says the opposite is very concerning. Thanks again for taking the time to explain the complexity of these issues. Simple answers would be great, but I'd rather have accurate ones. In a future video I'd appreciate if you can address if a lifetime of taking birth control pills plays any role in brain health or not.

    Agree hormone therapy, diet lifestyle are important. What happened in the WHI? Why are you not talking about Progestin and the issues with it? It is not the same as Progesterone. This study just once again proves why Progestin should not be used, period. ALSO, the hormones today compared to the early 2000'S are different estrogen was Premarin (from a horse)- not the same as estradiol that we use today. Also Progestin, not the same as Progesterone, in which we use today. There is not one single study with Progesterone that causes this issue, not one. Bottom line, you should have talked about Progestin, that it is synthetic, and in every study it seems to cause issues, including the WHI, increased breast cancer, increased dementia, The data is clear, Progestin is not safe, and it is not what we use in MHT today. This study and it's results do not surprise me,

    Thank you again for such a thoughtful review. How about a deep dive on the opposite effect of purposely removing hormones during continued treatment for breast cancer with Tamoxifen and Aromatase Inhibitors. There are millions, (CDC estimates), of women currently living with this therapy.

    I really appreciated this open and honest discussion, especially regarding hormone replacement therapy. I have an upcoming appointment for hormone testing because I am concerned about my osteoporosis and my risk of getting Alzheimer’s ( grandmother had it and now my 88 yr old daddy is in advanced stage of mixed dementia due to Alzheimer’s and vascular disease)😢.I am 66 yrs old and was his full time caregiver for 7 years.
    I eat the Mediterranean style, love to work in flower and vegetable gardens and have also started a more consistent daily exercise program to improve my bone strength.
    My weakness is I don’t get enough sleep. But the time I am in bed is uninterrupted (6 hrs)
    Thank you for all you are doing to educate us on Alzheimer’s etc etc. 9:38

    BHRT has changed my life...and brain. I am hoping that because this study of HRT was a "retrospective study," it is not accurate. They also did not study bioidentical HRT. On the topic of Alzheimer's: what are your thoughts about Rapamycin intermittant dosing?

    Such a clear and organized presentation of the information available. Thank you SO much team Sherzai! I am extremely interested in this topic and eagerly awaiting more conclusive evidence based treatment options. I follow a plant based diet, exercise daily and am working to improve my sleep which has been declining over the years along with my estrogen levels. I am 54yo and just lost my mother to dementia in December. I am currently struggling with poor sleep, hot flashes, brain fog, distractedness, and forgetfulness. I am very concerned about these symptoms and am considering HRT, but obviously don't want any negative side effects. I am really confused about whether or not it would benefit me currently and what form to take, if any. Keep bringing us great information :)

      I'm in the same boat. I follow wonderful menopausal doctors online. Seek out a menopausal physician who has additional training in menopause. Good luck.

    Thank you so much for opening up this much needed conversation. In addition to the various risk factors that you outlined, I think it critical that the psycho/social component must also be addressed. Post menopausal women also have an increased risk for anxiety and depression, their average income is lower, and they are still subject to the stresses of living in what is still a largely patriarchal social order. If you factor in epigenetic factors, women are only beginning to see less discrimination in almost every aspect of social interactions. How much of the suffering of our mothers and grandmothers is passed along to us. My personal observation in working with women with cognitive decline and Early Alzheimer’s is that to a one, the steep drop in their cognitive function occurred after either a prolonged period of stress, or an acute episode of major social defeat. I think this premise is also supported by the evidence that African American women have the greatest risk for cognitive impairment and Alzheimers and the related dementias. In one study, there was a direct correlation between the numbers of either personal or systemic racist exposures and the degree of cognitive impairment. I also think that a decision as to whether or not to use HRT must involve and understanding of the increased risk for breast cancer HRT infers. 50% of the breast cancers in women occur in women who are 60 or more years old. And the majority of those are estrogen and progesterone positive, meaning that those hormone fuel the cancer. I think that it is important to note that progesterone confers an increased risk for an invasive tumor pattern because it reduces a protein that holds the tumor together (E-Cadherin). I am hopeful that the conversation that was started tonight will continue and include these additional factors. Thank you again for opening this import discussion. The importance of more research on women’s increased risk for Alzheimers is absolutely critical.

      My MIL had heart failure, diabetes type II, and dementia. My FIL was abusive & treated her like his personal servant. She was NOT allowed to leave the house without him, plus cook plus everything! To the point, he’d even stand by her chair & order her to bed when HE was ready, even if she was watching her favorite program. All 3 factors played a part in her passing. To me, it was her heart failure. Her family called it Alzheimer’s or dementia. I lean towards… she was miserable & drank & ate her stress away because he was awful. Now my overweight SIL has taken on the burden of letting him live with them. There are days when she cries. I’ve told her to get him in to a facility. This is NOT her father! But my husband & his bother’s dad. FIL was so abusive that the boys, now men, were never allowed a self boundary. The old man is in 90’s & still healthy after Pacemaker & quad bypass over 30 years ago. I seriously think he will kill my BIL & SIL next. Stress is REAL… especially when there is no recourse.
      Dr. Peter Attia addresses the relative risk of Breast Ca vs absolute risk of Breast Ca. He’s furious that studies were done with equine urine and though they had the chance to study women on bio identical, they did not.
      I would love to know your opinion on bio identical? I used to be in medical field. I’m small with bird bones. Ob gyn has put me on lowest medroxyprogesterone with estrogen cream. He is worried about my risks of bone fractures. The cream is not supposed to have any effect to blood estrogen levels. But my liver still has to filter out the progesterone pill. I prefer the patch to bypass liver. Do you know where I can peruse info or research?

    I have read the book The Cure for All Diseases by Dr Hulga Clark. She has a chapter section specifically dedicated to Alzheimer's. In it she explains that 100% of the cases of Alzheimer's sufferers had aluminum buildup. Removing aluminum is a must. She also uncovered a parasite/fluke and solvents that are involved in the development of the disease. This study is incomplete without factoring in these contaminants and parasites.

      There’s likely more than one way to end up with alzheimer’s. Just because something is present doesn’t make it the sole cause.

    For women how does a history of hormone replacement therapy affect the statistics for Alzheimers.....Just came across your discussion! Was there any studies on chemical birth control? (the Pill)

    I'm start taken HRT after my last son means my son is 37 years old and the advise to have special diet, excersise, no smoke, no drink and be away for all sweet, inclusive Honey and carbohidratos. So I'm very happy my bone density is quite good condition thank God. And sexual life is very good. Aswell I'm takin levotiroxin every single early morning like 5.30am and every 2 years I have checkup and things are very good. And live life normal.

    Okay, so your expertise is brains, not really women's health so I recognize your really advocating for brain health not women's health, in general. So right here, you have a bias! Secondly, as a past researcher I want to see this research myself not just trust you or anyone speaking about the findings! And that's what we all should be doing, not just trusting you but start become critical thinkers for ourselves, come on people!? Ok... becus 50% of all these studies are down right WRONG in the 1st place, so it's like a 50-50 hit-miss coin toss really yet people take one study and suddenly make it like TRUTH and make bad choices based on one faulty study, like mass hysteria? One study or two or a few does not make a research truth okay! In an area of estrogen and older females in menopause it is seriously under-researched to begin with so a couple studies which may or not be even good research which we need to study this study to see for ourselves if it's even a solid study to begin with but it needs to be monitored and closely held to subsequent research studies that will either continue to support the findings in this study or dispute the findings in this Danish study. What truly scares me already is your presumption that this study is actually now the TRUTH? That's what really scares me these days about a lot of people and medical professionals. Ans yes, this is quite bizarre because previous beliefs held that estrogen aided in better brain health and prevented Alzheimers so we need more studies and thus the verdict is not yet out... and this is what we should all be stating nothing less nothing more at this point until tons of research is available regarding females and estrogen etc thank you!
