CN 11 would better be tested by asking to turn their head to one side against resistance. Shrugging the shoulders is not a good test as the trapezius is also innervated by the cervical nerves. So to lose the ability to shrug would mean the patient would have a very serious neurologic issue. (Might be paralyzed from the neck down)
