Is 4 Months Too Late To Start Tummy Time? What Parents Need To Know
Some parents might be asking, "is 4 months too late to start tummy time?" and the truth is that while tummy time needs to be started by day zero, there are a lot parents can do to fill in the gaps if a baby has struggled with tummy time or a parent hasn't spend much time placing their 4 month old on their tummy.

My baby could roll supine<>prone at around 4 months but has not been able to because his Landau is just too strong.. 😢 he just turned 6 months
HI Jesss! Thanks so much for the comment. While the Landau is totally normal, it CAN cause problems if a baby is not moving in/out of it (which is super annoying for both baby and parent!). The most comprehensive answers will come in my book tummy time and learning to roll which can be found here and comes with a free online bonus course with videos -- drlaurenbaker.com/tummy-time-and-learning-to-roll-book -- The Landau should; however, only mess with his belly to back rolling and NOT his back to belly rolling, so if little is struggling with back to belly rolling it could be that they have underlying tightness or once their body got bigger they weren't able to build the strength they needed to move a bigger body vs their small body at 4 months. Usually this happens when littles struggle with tummy time or if parents aren't taught that babies need sidelying play in addition to back/tummy play (I am trying to fix this!). My recommendation would be to grab the book and go through both the back to belly AND belly to back checklists to see what the movement blocks are + make sure to go through the mobility screens especially upper trunk and lower trunk rotation and side bending. Hey lauren my baby is almost 8 months since past 2 months he comes in crawling position rocks a bit n again lay down he is not moving his both hands forward..though he tries to moves his legs forwards but does not move his hand.
Hi Ajab! Thanks so much for the comment. At 8 months, we still have a little bit of time to learn how to coordinate our arms/legs with crawling. Typically, I am looking at crawling for most children 9-11 months. That being said, there are lots of things we can do to help your baby move forward, especially if they are getting frustrated! This exericse is perfect for working on hands especially if you do the rocking where the shoulders go in front of that hands, because they will need that movement in order to bring the hands in front of the body. I would also encourage you to go through the crawling checklist located here: drlaurenbaker.com/blog/learning-to-crawl-checklist to make sure there are not any movement blocks hiding under the surface so crawling is easy peasy! My one year old has been army crawling for just over a month. When I try put him on his hands and knees, he gets frustrated and straightens his legs. He can pull himself to stand and loves assisted walking, but I am not sure how to teach him to crawl on all fours.. what should I dooo?
Hi Tabitha! Thank you so much for your comment. I love that you are looking to help your child get into hands and knees crawling because it is SO IMPORTANT for baby development and it is the ONE milestone I am fanatic about parents encouraging their children to do and if they are struggling to figure out why and help them get into the position. Typically babies who have prolonged army crawling (greater than 2-3 months or after the age of 11 months) have weakness in their hips, arms or core. They could also have underlying tightness depending on how they are moving (do they pull with both arms equally with legs straight, do they bend one leg up and pull with the opposite arm but only on one side, or do they flex and pull with opposite arm/leg and alternate?). For parents of babies not crawling on hands and knees after 11 months, I always recommend seeing a pediatric physical therapist/physiotherapist in person if possible. I do totally understand that not every family has access to one in their area, though, so here are the things I would encourage: 1. download the learning to crawl checklist to see if there are underlying movement blocks, if anything is unchecked work on the corresponding action/exercise -- drlaurenbaker.com/blog/learning-to-crawl-checklist 2. Tall kneeling is my favorite exercise for babies who have prolonged army crawling for leg strengthening, check out the learning to crawl playlist for other tall kneeling ideas -- ua-cam.com/play/PLxYt0JBWx03QAXXe0HL5A5WnN9Htyjx4K.html 3. In the learning to crawl playlist also check out the baby plank to make sure that your baby can tolerate pushing through extended arms for at least 30 seconds but ideally for 1 minute while being heavily distracted (this exercise is hard!). pulling to stand and standing/cruising is MUCH easier than crawling, believe it or not, so there are a lot of babies who walk but skip crawling; however, it is not ideal and often those children will have struggles with handwriting and potentially reading/copying down from the board that their same age crawling peers do not have so I always recommend working on crawling through pre-k for any littles who have skipped or struggled with crawling on hands/knees. My baby is 8 months old. He is on all fours but neither sits nor crawl.
Hi Megha! That is awesome that your little is already getting into all fours (even a touch early, but also not uncommon to see -- usually happy if babies are getting into all fours at 9 months and crawling hands/knees by 11 months). Since you have time with crawling (9-11 months is totally normal) I would focus the most on sitting. We want babies to be stable sitters by 8 months (ideally) or 9 months (if not by 9 months it is considered delayed but often can be due to muscle tightness vs scary reasons). My biggest recommendation would be my newest book which just came out on Monday (just in time!) which will help you focus the most on sitting as well as check to see if there is underlying tightness or what blocks your baby might be experiencing with trying to sit on their own. To be honest, of course, the main consideration is time since I am not a full time creator and also run my own business and am an author writing four books in four years (two more to go!). Hi Lemek, thank you so much for your comment. I want to make sure that you are calculating your child's adjusted age until they are age two (this will help you understand the specific month that they are developmentally appropriate for). Here is an adjusted age calculator that you can use: starship.org.nz/health-professionals/calculators/corrected-age-calculator/ If the baby is adjusted age is over six months, our goal is rolling back to belly and belly to back. With head control our focus is whether a baby can lift their head to 90 degrees while on their belly and whether they are spending 60-80 minutes of tummy time per day with a goal of 5-10 minutes of sustained tummy time without resting their head on the ground. The baby can also be working on supported sitting depending on level of head control. Some babies who are born premature simply need more time and/or support, so if a pediatric physical therapist is available in your area having one on your child's team would be really helpful for you. Some infants that were born premature have additional diagnoses or had trauma at birth (brain bleeds, limited oxygen, etc) that can impact development as well. The best thing you can do is focus on developmental milestones and bring up concerns with your Pediatrician who can do additional testing if warranted. My recommendation right now to help you get the information you seek is my first two books in my proactive play series: learning to roll and learning to sit, since they are both helpful for developing head control, rolling both directions, and sitting. At 12 months old, if a baby is not yet crawling on hands/knees we do recommend families contact a pediatric physical therapist in their area for support since it is past the recommended age range of 9-11 and more could be going on. It is awesome that your baby is able to get into sitting by themselves (9 month milestone) as well as roll in both directions back to belly and belly to back (6 month milestone). Babies who struggle to push through their arms often have weakness in the upper trunk/arms/shoulders and often struggled with tummy time or did not spend the recommended 60-80 minutes per day on their bellies. Since your little is not yet crawling, my recommendation would be to start with the crawling checklist to see what areas the baby has a skill gap in, then you will also get access to the free crawling online course: - Crawling Checklist - drlaurenbaker.com/blog/learning-to-crawl-checklist If a baby is not crawling on hands/kness, they often do not have the opportunity to develop the strength in the hips/lower trunk that they need in order to walk. For walking we are looking at 16-18 months as still within the normal range, and often babies start walking on their own approximately 3 months after crawling on hands/knees becomes their primary mode of mobility. Hello , I’ve just purchased your book can’t wait to try the exercises , my baby started rolling quite early and was regularly rolling back to front from around 4 months. 5 months he started sitting and the rolling stopped, I’ve really upped the tummy time doing 30 minute sessions and he’s started doing belly to back at 7 months. I have always prioritised tummy time and he’s tolerated it really well so I’m a bit stumped as to why he stopped, he’s also just started bum shuffling! While in tummy time he does do the landau reflex which I wasn’t sure if that affects rolling. He does push up through his arms, reaches for objects etc and will pivot but very slowly and rarely. He also sleeps on his side so rolls to that position at 4/5 months he was sleeping on his front. I just don’t want to be affecting his ability to roll and crawl. He’s rarely in a container and we always play on the floor!
@@jessicadale1086 hi jessica! Love how much you are doing and I think the book will be hugely helpful. Start with the checklists (back to belly first) but likely it is a mobility issue (upper and lower trunk rotation). Make sure to sign up for the free course to view the videos too. Bum scooting is not ideal and can be from weakness in arms or tightness in the trunk so those are definitely the two areas I would look into first!

Hi Heather! Thanks so much for the comment and being patient with me as it has taken longer than I'd like to respond. This technique is called Total Motion Release which is based on the strain/counterstrain osteopathic principle to increase motion (different than traditional stretching where we would pull on the muscle, creating a stretch, hold for 30 seconds+ and then do both sides). Babies cannot always relax through that stretch feeling, thus at times making traditional stretching less effective. The TMR protocol works differently so you only want to do the easy side based on its principle of shortening the muscle to neurologically and physiologically relax it, then retrain the body through vestibular input and strengthening to increase mobility. Hi Saumya! For tightness in the arms, it could be due to tightness in the trunk. This video will show all the directions that you can test to see if one side is more difficult than the other, but the tummy time and learning to roll book will be the most comprehensive to show mobility exercises and positional releases. That is what I would recommend if your little is still showing signs of tightness or is struggling with tummy time, since tightness can impact a baby's tolerance or ability to push through their arms on their belly. My baby is now 3.5 month old he has ancle clonus sustained when I found he was 6 week old but now it drastically reduced it happened mostly when he is in sleep when I elict but mostly not some times happened. He had good head control during tummy time. He can suck his both hand grab toys when I put him in middle of his chest but I feel some stiffness in his hand. He used his left hand more than right which I feel he has left side preference in initial but it reduce now he can move his neck both side he can turn right side too. Hi Saumya! Thank you for the comment and being patient with me as it has taken me longer than I usually like to respond. Ankle clonus in early infancy for a few beats can be considered normal, but > 10 beats or sustained clonus at anytime would be considered abnormal. The fact that is has decreased is promising but if you notice it continue I would have your child's pediatrician assess that including their reflexes to see if they note anything atypical. Clonus past seven months is also uncommon and would be considered something to be evaluated, as well. Stiffness in hands and using one hand more than the other are things to continue to monitor as we do want infants to utilize both sides of their bodies equally. Sometimes stiffness can be muscle tightness and sometimes it can be due to something else going on in the system or trauma from birth, etc. This is another item that a child's pediatrician can look further into. At now 4.5 months, we would want a baby to be able to lift their head to 90 degrees, spend 10 minutes in tummy time without difficulty on the floor at a time, spend 60-80 minutes total per day in tummy time, and start showing signs of rolling back to side and/or back to belly/belly to back. We would additionally want them to tolerate sidelying play on both sides. Hi Mina! thanks so much for the comment and the patience as I am catching up on comments. It is not typical for someone to feel pain but it could be due to a variety of reasons. The lower left side (the lower quadrant from a medical term if you want to google images of exactly what I am talking about) is where the descending colon connects to the rectum, which is where poop can get "stuck" -- that would be my guess on why it could be painful. That being said, inflammation can cause irritation in a lot of areas, which can be due to leaky gut so if you do find that it is painful (or continues to be painful) my recommendation would be to contact a functional medicine doctor that specializes in gut health (bonus if they specialize in gut health and hormones) because they are going to be the most knowledgable on why it is painful. They may do tapping/percussion on your abdomen to determine if the cause is gas vs stool (the percussion sounds different) and they can tell you which structures they are palpating and why you are painful. I highly recommend a functional medicine doctor for any gut health related issues (I am going to one myself and it has been wildly eye opening and helpful!) I hope this helps Lauren

Hi Defne! Thank you so much for your comment and for your patience as it has taken me longer than I like to respond! Hypertonicity is tough because it is going to kick in whenever a kid is feeling unstable or can kick in when a child is trying to activate one muscle group but globally activates others instead (I see this most when a child is trying to activate their lateral muscles but instead globally activate their extensors). I do not normally avoid activities that could increase tone if that makes sense, but I do want to try to find a medium where a child can work on the muscles they are using less to increase the dynamic control that they may have overall. So I wouldn't personally avoid using the ball, but the way in which you use it might need to be adapted (smaller motions, flexibility first then strengthening, etc). I hope this helps! Lauren

Hi Andrea! Thanks for the comment. Typically crawling up and down stairs is later than crawling on a flat surface like the floor, although it can be started earlier than the master age of 15-16 months, I would wait to introduce until you see your baby pulling to stand from a couch or chest high surface as they’ll need that half kneel “will you marry me” pose in order to be ready to crawl up stairs. Crawling down stairs will be more difficult so not uncommon to introduce crawling up and then needing to assist crawling down - can help by assisting babies to go feet first down couch cushions etc. i hope this helps! Hi Beyond the Veil, if a baby is over five months and still struggling with head control then that is a concern that should be directly brought up with a child’s Pediatrician/doctor. If a child is 0-5 months, head control is best developed in tummy time, back play, and sidelying play. If there was trauma at birth or concerns for an underlying condition, the child will need to be seen by a provider for further evaluation or treatment. Three months is a bit on the early side but is not a concern unless a baby is utilizing tightness and tension to roll instead of their muscle strength and flexibility. Most babies start rolling 4-6 months but some babies can roll 3-6 months (typically belly to back but could also see back to belly). I would go through the rolling checklist to see if your baby has everything they need for mastering rolling just in case tightness is contributing (not as a worry but more like an experiment for optimal development if that makes sense!). Hi Kayley! SO GLAD you got the book. I hope it is helping! Likely trunk tightness is causing your little discomfort and possibly tightness in the front of the neck. If you have not already, I would look at the stability ball backbends/guppy pose and upper/lower trunk mobility sections specifically in rotation but also side bend. You could do the upper trunk rotation in sitting or on the back but likely the upright hold positions would be most appropriate. Thanks for the video. We are from India. My 16 year old daughter has always been a toe walker. Orthopedics say its due to short archilles tendon. Few suggest lengthening surgery and few have sugegsted botox injections. We are not 100% convinced with either. She can walk normally when she realises it. But over the years she has constant pain in calf and toe numbness too which I am mostly concerned about. Is 16 too late? What can i do as we did PT and even AFOs for 2-3 years before Covid and it didn't help at all. Thanks
Hi Bushra, thanks so much for the comment. I would continue to be intentional about working on toe walking even at 16. There are so many different aspects to toe walking that it is a really difficult diagnosis to fully resolve but worth the effort to maintain an effort toward. Here are a few additional ideas that may or may not be in the video (sometimes it is hard to remember!) - The toddler squat: this is my favorite for strengthening and lengthening those ankle: ua-cam.com/video/dhsJdtHAj94/v-deo.htmlsi=ycV8EMOYP7QJlin0 Sensory sole inserts: ua-cam.com/video/eWOuoReZHPA/v-deo.htmlsi=0Rj9g7QKwfGuwO1i Vibration plate exercises: some people need more input in their feet and vibration can be a great option. I use a vibration plate off amazon but I am not sure if it is available in india. If you have access to a clinic with a hypervibe that would be ideal. Here is the vibration plate I own: kit.co/drlaurenbaker/pediatric-physical-therapy/6225505-lifepro-waver-mini-v An outpatient physical therapist might also be able to complete ankle mobilizations including the anterior-posterior glide of the talus which might assist. Tendon lengthening procedures are extreme and may or may not solve the underlying cause. If she can get her feet flat with intention, a tendon lengthening procedure will likely not solve the toe walking as she has enough length at the tendon to be foot flat, that further lengthening is likely not helpful but each child and family should definitely discuss these with their provider’s recommending that care. Bushra, the numbness could be due to neural tension in the nerves in her legs. I believe I talked about sciatic nerve glides in the video but if not there are a ton of good sciatic nerve glides on youtube to try, the other one I would recommend to try is the femoral nerve glide. My baby is 6 months old he can get up on all fours and rocks back and forth since he was 5 months old. He ends up moving around on the floor by pushing with his arms.

Hi Sam! Yay for getting onto all fours. Sometimes babies as early as six months can get into the position but we don’t necessarily expect them to hands and knees crawl until closer to 9-11 months. If a baby crawls earlier, that is totally fine AS LONG AS they also mastered the milestones before such as rolling in all four directions and sitting by themselves. If your baby skipped rolling in any of the four directions (both right and left for back to belly and belly to back) then make sure to grab a copy of my book Tummy Time and Learning to Roll - drlaurenbaker.com/tummy-time-and-learning-to-roll-book - while it isnt super common I have had babies who crawled without rolling and it can inhibit future milestones as well as sleep and head shape so ideal to make sure babies are doing all the things before, if they move on a bit early. All four rocking is developing the weight shifting needed for crawling but likely he is still developing the strength for holding all fours before moving onto full crawling which is totally age appropriate. Army or belly crawling is awesome as long as it develops into hands and knees crawling by 11 months! Great video. Thank you! Any pointers for a 9 month old girl that locks her lower legs straight and won’t put her arms down when attempting to assuming the positions shown in the video? She usually screams and cries. Every time she’s placed on her tummy she is very adept at rolling to her back. She will sit unassisted for about 30 minutes at a time. She’s able to stand and support herself against a stationary object but hates to crawl.

Hi Fathima, thank you so much for your comment. I appreciate that you are looking for answers for your nephew and I am happy that the hydrocephalus is no longer impacting the pressure on his brain. Unfortunately, hydrocephalus even if it is less of a concern now, could have impacted the brain (any time there is lack of blood flow brain injury can occur and pressure from fluid can cause lack of blood flow or pressure injuries to the brain tissue itself). That being said, babies with brain injuries or hydrocephalus can surprise us all the time and I try not to look at the big picture regarding scary diagnsoses when looking at development and only work on the next small milestone, and once we get there, the next one, and so on. My hope is that your nephew has a great team of doctors that includes a pediatric physical therapist or physiotherapist to help build strength. As he has a more complex case it would be necessary for someone in person to work with him directly. Hi Raju, unfortunately there are not any specific shoes that can stop toe walking since there is a lot of things going on that might be the cause of toe walking. These inserts (not sure if they are available in india) may help. Additionally taller shoes such as high tops where the shoe goes above the ankle can give more support and make toe walking more difficult, but are not a cure. Hello mam, my son is 3 yr old now he is not walking yet he started crawling and sitting in a w posture at his 2 yrs ...we continue giving physio therapy ...when we make to walk he is walking with his toes and bend his knees .. what are ythe effective exercise to do mam for quick better results
Hi Rvp! Thanks for the comment and for the patience, as it has taken me longer than I would have liked to respond. I am so glad that your son is in physical therapy in person! That is going to be the absolute best thing for him. I am assuming that there is an underlying diagnosis which is impacting his ability to walk more fluidly. I am a bit confused but hope this is helpful regardless. My understanding is that he is not yet walking on his own, but that when you help him walk he is walking on his toes with bent knees. For those who have underlying diagnoses it is super tough to share information that could be helpful since I have not seen them in person. That being said, I will share as much as I can with the information I have. I would also recommend asking for more explanation from you in person PT as they will likely have even more insight since they see him in person. If a child is not yet walking on their own, they often lack strength in their ankles, knees, hips, and lower core. Your son is building these muscles through crawling (hopefully on hands/knees otherwise I would focus on that!). If a child is not able to support their weight through their legs in standing, they are going to lock out their ankles (by pointing their toes) or lift their legs to bail on standing. Before a child walks on their own, they stand supported (holding onto furniture) and then cruise (walking while holding onto furniture) then they need to stand without support before they can then move onto walking while pushing a push walker/toy or walking while holding onto a parent's hands. For children who are older and still not yet walking independently, they often require assistive equipment such as a reserve walker or gait trainer to give additional support. My recommendation would be to utilize the toddler squat strengthening position video linked here: ua-cam.com/video/4NS0gQn_SOo/v-deo.htmlsi=olUkcOqXd8wsRwyO (get it approved from your PT because they know your son best). This is a great core, lower trunk, and ankle/knee/hip strengthening exercise that you can modify to best meet your son where he is. For you and others who find themselves in this situation I am going to respond to the question and then I will also add a few other resources that might be helpful since your child is now 9 months old. If your child is 9 months and not yet sitting, pediatric physical therapy is also recommended to help them sit up ideally in person. As far as the heart condition, that will impact development potentially but is something that a surgeon and/or pediatrician would have to share more information about. Development requires nutrition as well as good oxygenation and bloodflow so a heart condition could impact development but not always. The way I am reading it, it seems that your son was previously able to sit at 10 months, but now at 11 months is struggling. This exercise is great for core strengthening and head control, but if sitting is still a concern, I would reach out to your pediatrician or physician as babies who are struggling with sitting after 9 months may have something more going on and we want to rule that out.
