The Cavemothers (there’s another dead one chained up so there’s more than one lorewise) use humans to get pregnant. They also use human flesh to feed their babies. So they basically rape the men in the dungeons then eat them. The area with all the cells (level 3?) has open blackness leading down to these tunnels and cave gnomes flying about. I think it’s highly likely the prisoners in the cells in that area were all forcibly bred by Cavemothers and then eaten. These games have some really good environmental story telling.

When you sacrifice the guy in the altar it gives you affinity with All-mer (A god in this universe) It usefull to get affinity cause it makes possible to you buy skills in altars using souls. You'll figure out the remaining
