Jay's Story
Meet Jay, whose twenty-year health journey reflects the experience of countless Fat patients in rural communities. From an old motorcycle injury to high blood pressure, their story illustrates how weight-centric medical care can lead to years of frustrated consultations, missed diagnoses, and deteriorating health.
00:18:44 - What does weight inclusive care look like?
In this episode, I break down how weight-inclusive care changes everything - from the first examination to long-term treatment plans. I share how proper medical investigation, understanding of weight cycling, and compassionate care can transform both patient outcomes and the doctor-patient relationship.
00:27:19 - Everything You've Been Told About Weight Loss Is A Lie
Diet and exercise do NOT reduce the risk of developing OA of the knee according to the PROOF study. Today's study is entitled "The role of diet and exercise and of glucosamine sulfate in the prevention of knee osteoarthritis: Further results from the PRevention of knee Osteoarthritis in Overweight Females (PROOF) study": pubmed.ncbi.nlm.nih.gov/26806185/
00:31:13 - Ask Me Anything
I was asked to weigh in on the weight inclusive treatment of high cholesterol (nice pun, by the way!)

What Is Health, Really?

Health isn’t a one-size-fits-all concept. It’s as diverse and dynamic as the people who experience it. Raven's Story

Raven's story is one that I imagine many of us are intimately familiar with. Raven is a 34 year old graphic designer, A plus student and all round badass who has been trying to shrink their body their entire life. Jude's Story

Jude is Fat, exahusted, and ashamed. They've been so tired lately they finally bit the bullet and visited their doctor. Of course the doctor told them to eat less and get some exercise. But they also mentioned a sleep study. But Jude can't possibly have sleep apnea because that's just another thing everyone will blame on their weight. What's Jude to do?

There's no such thing as the Amercian Dream

Surgeons are lying liars who lie

It's amazing what some surgeons will say to get out of performing surgery. Which is weird, because they're supposed to like performing surgery. Otherwise why become a surgeon? 4 years ago, Asher failed his final diet. More accurately, the final diet failed him. 3 years ago, he came out as transmasculine. Two years ago, he uprooted his entire life and moved to Scotland. This time last year, he realised he was neurodivergent. In may 2020, he was writing a weight loss blog. A lot has happened since then, so he figured it was high time he re-introduced himself.

I don't know how this popped up in my feed, but I looked you up because your statements in this seemed dangerously misinformed-- you are no longer a doctor due to losing your medical license earlier this year, which checks out. While weight cycling is bad for the body, you're acting like that's the only option. And it's simply not true that weight cycling "almost always" results from intentional weight loss-- it appears you are only saying that it does because that's just been your personal experience. Diets don't work long-term, but permanent lifestyle changes (i.e. stopping overeating, eating healthy foods, and exercising regularly) do.

I don’t like leaving comments on UA-cam because it feels just so public to have my name out here in commentland, but why did I CRY through watching this? Your journey strongly resonates with me on many levels. I found your content in late 2020 or early 2021 as one of my children was dx with an eating disorder in late 2019 and I was beginning the task of unlearning because I knew it was integral for me to do so for the benefit of my children. I had been challenged to do so by other eating disorder caregivers in a support group and someone had recommended your content. It felt like a life and death situation for me to unlearn and relearn for my child’s survival. I was desperate for her sake. So you were/have been one of my guides to unlearning all I thought was true regarding health. But I did not know until today that you were also going through this unlearning journey at the same time, likely mere steps ahead of me. I know you don’t need my empathy but I can’t stop it; I have it for you and others who pave this path. Your impact on the world is enough! Those who are ready to unlearn will. The rest will challenge you and accuse you of things, it seems, and all that does is garner more attention towards the inaccuracy of information being sold to us by our governments, healthcare systems, and education systems. There’s more to my journey that finds connection to your journey, but I’m not comfortable with sharing anymore so publicly. I hope your job continues in this way to the benefit of those in need of your service and for the benefit of you and your family. Happy belated trans birthday to you 🎉

Others in the health world think I'm a bit fringe, and just promote the 'no dieting' because I'm big. No-one has stopped to ask me why. No-one. NOT ONE person. They are all a little embarrassed, I think. I'm sure they think I go home and gorge. And sometimes I promote that thinking, as I am so over NO ONE asking me anything about my journey. If I lost weight they would all be on to me like a rash, asking me how I 'did it'. Until I regained the weight, in which case they would mutter behind my back about my fall from grace. Your body actually does get the difference. In fact you gut flora play a major role in what you crave to eat. There are colonies in your gut prefering carbs/fats/proteins. So it is a lifestyle change and futher it is a bodily change. Been there done that. I was a sugar tooth and now I am craving more meat and fat. It takes time and dedication to do the switch but it can happen. And of course that involves changing what you eat. Why its not a diet? Because i still gain and lose weight depending on life happenning but I do find it easy now to maintain it at a good level. Cards and processed sugars are shit.

bmi is just a measurement that tells you how much weight your frame can safely carry. people with very large/unnatural amounts of muscle like alan ritchson from reacher also start developing health problems as their bmi goes up. weight around your chest and neck can put pressure on your heart and lead to sleep apnea. carrying too much weight in gereral will cause strain on your back and knees, its the same for everybody, it doesn't matter how fit you are, or what percentage of bodyfat or muscle you are carrying.

Collecting BMI data is just as useful as collecting eye color, or height data. It’s not readily useful in any particular way to the individual but on mass these, along with hundreds, or thousands of other metrics (or datapoints) are useful to find correlations which of course do not equal causations. I have blue eyes, and if there’s a higher correlation between having blue eyes and getting cancer that could either be coincidental, bad data, or actually connected in some way so I should get screened more often to align with the higher percentage of getting cancer compared to people with brown eyes. BMI is just a datapoint but at least it’s a datapoint you can change if you believe it would be beneficial to you, unlike having blue eyes which you cannot. I like that we collect datapoints on aspects within our control.

Nearly 100 years of research shows that only a very small percentage of people can lose weight and keep it off long term. About 2-5%. So BMI is not very changeable via weight loss. The most likely outcome of intentional weight loss in the long term is weight GAIN. We only pretend that weight is a modifiable factor (down) so the diet industry can make money.
