i had something similar to this happen when the dlc first released, i realised it was a bug with stage aesthetic, idk if you use it, but it is likely a mod issue, but if youre playing vanilla i have no idea
It was full vanilla lol
With the randomness of the director this is entirely possible to happen with bad luck but is very likely a bug as the teleporter was also not normal.
I feel like it was most likely a bug with Devotion, I think I just got lucky with the teleporter though lmao
