    Thank you for all the information! I've had a dual chamber for 5 weeks now. It has taken care of the bradycardia but now i have the fast rates. Have had Afib at 229 and my brady was in the 30s-50s. Feeling better but still not quite right. This video has answered so many of my questions.     Dear Pro. JOSHUA COOPER, I really really appreciate your unbelievable video. It's so professional and vivid. I think it's definitely helpful for Chinese electrophysiologists. However, they can't use the UA-cam. I would like to know if I can copy the video and share it with Chinese trainees on Chinese websites. Anyway, thank you very much again. I need to review this vedio over and over like a reentrant VT.

    What type of exercising if any would cause the leads to break? Push-ups? Pull-ups? High Overhead stretch as in Bikram Yoga? Slalom Water skiing - getting up - arms pulled forward to skiing a course (one handed turns)? Thank You for your responses!

      Pacemaker lead fracture is difficult to predict and there are no specific activities that are predictably associated with lead fracture. The anatomic relationship between the pacemaker leads, the clavicle (collar bone), the ligament between the clavicle and the 1st rib likely plays a role in lead fracture risk, but that has more to do with the lead implantation and patient anatomy than specific activities. And so, both for quality of life reasons, as well as no data to support activity restrictions, there are no prohibitions on any particular type of exercise with a pacemaker in place (including no restrictions on push-ups, pull-ups, overhead reach/stretch, slalom water skiing) - but I envy the person who is this physically active and I must vow to do better myself!! :)

For me, it is always a struggle to understand what actually happens and why it looks like that on an ECG, and you make everything clear. Please continue to upload more videos! It would be amazing if you could upload a video about SVT and explain the different definitions, for example: what is the difference between atrial tachycardia and focal atrial tachycardia? Or is SVT the same as atrial tachycardia? And of course the ECG patterns of each SVT. THANK YOU!

    Very interesting. My question is whether there are any differences between treatment/tips for males and females? Women have a different torso to men, which may mean that different muscle groups in the chest need different maintenance.

      Pacemakers are small, compared to local muscles of either men or women. No specific gender differences in recovery or maintenance. 👍

    I was born with WPW. It only was known to me around age 48 as I started menopause. Now, at age 80, I haven't had any irregular fast rhythms for several years. I wonder if the accessory pathway shrunk, like everything else as we age. Also, my resting heart rate is lower than when I was younger, but I make it a point to workout every day. I am a coach at a local gym.

    I have had PVCs for 50 years but as of late they are getting worse. I have Parkinson's and when ever I get stressed out which is often lately I get them bad. I'm having them right now as I type. It's driving me crazy and my cardiologist says don't worry too much about them Well I am because they are getting uncomfortable. I have never smoked and never drank much alcohol. Any suggestions ?

      @@dr.joshuacooper-arrhythmia9917 I had a dual chamber pacemaker inserted almost two years ago, so now and then I search for pacemaker related videos. I must say you did answer some of the questions that I had asked at my pacemaker check last week i.e end of life with a pacemaker. As a former nurse I like to keep up as well.

    I'm a 57yr old male, I had PSVT with WPW and AFIB. I couldn't get it on an EKG for 40 years. I've worn so many monitors, I think I used every single type many times. Then about 5 years ago I wore an extended time (30 days) model with a cell connection to a computer system somewhere. The very last day I had it on it started to fall off. I was very frustrated and removed the pads from my chest. Wouldn't you know I then had an episode of SVT, lightheaded and not feeling well I scrambled to put the pads back into place. I was lucky to get a graph of my WPW and had an ablation a few weeks later. 40 years of vasalva maneuvers, carotid massages and cold showers to control the episodes was over. I was so happy, so thankful to my cardiology team. I'm left with AFib now and treat it pharmacologically qith some success, I still get AFib issues but we adjust medication to set it right again. I've been cardioverted 2 times over the last 5 years.

    Thank you so much for explaining this! None of the ER doctors I've seen over the last year or even my doctor who did my SVT ablation didn't explain it to me this well! They all just told me it's normal and not dangerous. Its given me a ton of anxiety because I kept telling them its a skipped beat and not an early beat but finally I understand why it feels like a skipped beat. You also explained why I cough sometimes when it happens which I never even really thought about before.

    Oh my! I've learned so much more from you than from the device tech and cardiologist , but its hard to learn much from a 5 minute doctor appointment. I've only seen the cardiologist for 5 minutes in the 15 months I've had the pacemaker that was 5 months ago when he told me one of the leads was not working and that it broke a few months after getting it. He doesn't want to run any tests besides remote interrogations to see if the separated or why it's not working.

    Why would a pacemaker not send a signal when the patient is having an arrhythmia? Why would it not show up on remote interrogation? The atrial lead is not working ,could that be why? I know the remote interrogation is always done during the middle of the night and I've never felt an arrhythmia when I lay down. I feel so confused. They don't tell me much of anything.

    PVC and anxiety come together not sure which one first for me, but I got all results from heart ct scan - heat monitors- echocardiogram . More time you wait for all appointments and the symptoms got more bad because of the anxiety ! You more worried and stressed, finally almost 2 months of waiting went the ER 2 times and went for Docter too many times I was so done 😩 so now I’m on new medication hope it help ! Feels for everyone who in this situations hang in there!

    I am congenital bradycardia patient. I had complete heart block. Got pacemaker few months ago but since i got it. Life is not normal. I'm 40. Get tired and feel shortness of breath. Sometimes feel slight pain and pressure on left side and left arm. Is it normal after pacemaker?

      There are several reasons that someone might not feel well after getting a pacemaker for heart block. Much of it has to do with the way the pacemaker is programmed, and lead placement in the heart can also impact how the pacemaker functions. You should see your EP doc and let them know you are not feeling well so that they can help figure out solutions.

    Thank you for taking the time to put this video together and sharing it. My family wants a clearer explanation than what my doctors could give them. This is super clear. I just received a double ablation and a pacemaker/shocker combo 5 days ago. During the EP Study, I was thrown into VT twice. Thus the double ablation and shocker. 👍 For what it is worth, I haven’t felt a single PVC since the surgery. I think I was very lucky. Before surgery, I had full time bigeminy, and randomly it would line up 2 and at the worst times, 3 in a row for almost a minute. When those happened, I felt like I was on death’s door. 😢 Talk to your doctor if you’re feeling irregular heartbeats. It’s not worth taking a chance. I survived a, 100% occlusion main left artery, heart attack 7 years ago, so I’m super fortunate to be here today. I’m a little stubborn, and I refuse to lay down when things are going bad. 🤷‍♂️ I literally would not sit or lay down during my heart attack until I was in ICU. I then knew they had me. They lost me once, but was revived. Sorry this is so long, to be honest, it hurts my mind even talking about all this. I guess I’m finally letting it all out. Nice to meet you all! 🤣 And please take care. People love you and don’t want to lose you. 🥰

      Ah, i think you're referring to the case example at 1:37:45. In this case, earliest atrial activation is *neither* in the center of the heart near the AV node, *nor* over leftward in the left atrium near the distal CS catheter. Instead, earliest atrial activation is way over in the lateral *right* atrium, nearer to the HRA catheter and further from the septum/AV node. And so this is not central activation, but rather it is "right-sided eccentric," meaning originating at the lateral right atrium. I realize I am using the term "eccentric" differently from what people usually say when they are referring to the earliest signal being over at the lateral left atrium (AKA left-sided eccentric). But both lateral right atrium and lateral left atrium early activation can be seen as "eccentric", meaning not concentric where the septum is earliest. You are correct that the coronary sinus catheter alone has a concentric pattern, but when you think of both atria in their entirety, we also have to take into account the possibility of non concentric activation way over on the right, where the only recording catheter we often have is the high-right atrial catheter.
