I don’t know if your machine allows it but if you can zoom the lateral tricuspid annulus and then M-mode it you’ll get a much easier trace to evaluate.

Hello! Pricing of echocardiograms varies quite a lot, depending on what the reason for the echo is (how complex it will be), who is performing it, and what country and region you are in.

I have had many dogs and several of them are shelties. Yes, she is a very good girl. The machines are all familiar to me as well as the views after 40 years in human Echocardiography.

A great little video for the more advanced scanners. This view is great for seeing the left auricular appendage (for flow velocities in cats - correlates with the risk of thromboembolism) and also the pulmonic veins entering the left atrium so you can measure pulmonic vein flow for diastolic function assessment.

So glad I watched this two days ago. Yesterday, loaner machine set up all wrong for me. First 3 minutes trying to work out which way round the probe image was set. By the end of the study I had diagnosed my first tricuspid regurgitation. Thank goodness it was a ragdoll.

Totally agree Catherine🙂 Wise words. I was a referred a wee dog with a box shaped chest, and concurrent respiratory disease the other day. We had to play ‘where’s the heart??’ Particularly between breaths. We never achieved an adequate left apical view. Enjoy your (modest?) summer. We have 2 students (one from Hong Kong the other from Germany) visiting us in New Zealand. They have sacrificed their summer for a kiwi veterinary experience - that’s dedication 😀

I love the term 'scanning blind' ! I find a big problem is that the dogs that we need to do subaortic view on (e.g. boxers) their ventral costal arch is too tight to get the probe in - any tips?
Do you have someone bringing their back legs in for you? Sometimes it can be a matter of waiting long enough with a low frequency probe on the abdomen, the dog gets bored and heaves a big sigh, and in that moment you get a couple of waveforms on screen! I'm half joking, but getting the dog to sniff for a treat can also sometimes give you the lucky break you need. In such difficult cases it may be that the apical view gives you the best alignment - thank goodness for the option of multiple views in echocardiography!

Another one is pulmonary regurgitation. I often get pulmonary valve regurg... But how to asees the severity or how much regurg is considered normal?
A lot of patients will have trivial or mild pulmonary regurgitation, and it's a sign of a good echocardiographer to identify it on most patients! You are welcome to email me a few of your cases, and we can rank them together in terms of severity so you have pictures from your own machine to refer back to in the future.

Thank you Catherine. But if we don't have cw Doppler, how to quantify the jet? How to assess the severity? Is there a classification like mitral valve regurgitation?
You need to consider upgrading your machine to one that has CW.
You can make a visual assessment using colour Doppler (I've briefly mentioned jet width to LVOT diameter in this video, but this is only semi-quantitative), as well as looking at other signs, such as a dilated or hyperdynamic left ventricle (depending on whether AR is chronic or acute). In severe AR the mitral valve may also close prematurely.

Thanks for the great question, dmantsio. The orientation of the probe marker in relation to the dog can also depend on which side you have your on-screen marker. I will make a video next week, to answer your question properly.
