We wrote the book on software failure modes effects analysis. We invented the methods for assessing software reliability before the code is written. We analyzed thousands of software failure events and categorized by failure mode and root cause. Our founder, Ann Marie Neufelder, is the chair of the IEEE 1633 Recommended Practices for Software Reliability.

The final step of the software FMEA is tracking the failure modes/root causes to closure and updating the likelihood if/when the CDE is fixed, controlled and a test procedure is developed to ensure that any failure is detectable in development/test. The critical items list can be generated by hazard number or by severity level or both.

In this step the mitigations for each of the relevant CDEs are assessed. The migitations include adding or changing specifications, design, code and/or test procedures. Mitigations are not analyzed from the user point of view with the exception of identifying changes to the user manual. Mitigations are for the development and test team.

In this step the likelihood is reviewed. Requs Software FMEA auto calculates the likelihood of each CDE based on the answers provided in the survey tab. The analysts can override these if there is justification. The compensating provisions are also assessed. In our example, there weren't many compensating provisions because the user cannot be assumed to be either home or looking at the thermostat.
