You're welcome! 😊
Why are conjunctival cysts located near the corneal limbus?
Good question, I don't have a good answer and certainly not an expert in this matter. If I were to guess, microtrauma causes the invagination of conjunctiva and this forms the cyst. This is likely also to an extent due to exposure to UV light (Sun).
Two eyes on the same day is new to me
好恐怖!!!😮😮😮
🎉 can panoptix halo starburst go away?
Yes, it can. In the majority of people, through a process called neuroadaptation, the brain can adapt to the signal from the eyes and optimize the image. This process can take some time, maybe even up to 1-2 years.

I had multifocal toric lens in my eyes
Fantastic choice! They will last you a lifetime!
@@JJChopsticks in my right eye I have Restor multifocal toric IOL, in my left eye I have Panoptix multifocal toric IOL
@@raznologija745are you happy with the quality of them?
This video is everything! I wish I’d seen it prior to two visits with a cataract surgeon and a second opinion! I also wish I wasn’t in Texas or I’d make haste to see you! Love your style. Explanations are so good. People ought to pay you to use this video with their patients. Thank you. PS-Is part 2 a future video?
Haha yes, I am in the process of making a video about risk with RLE. Thank you!
Any updates on part 2?
You are amazing
At 3:20, you inserted the 3-piece IOL upside-down :)
I must have loaded that one myself 😁
Can you tell me the reason of iol exchange
I believe we had difficulty adjusting this lens and patient wanted to start over with the adjustments. Great question!
Que gran catarata
Do you know what brand/size lid speculum that is? You always have really good exposure/lid retraction on your videos. Thanks in advance
You bet, it is epsilon special order E1-206, or when backordered Ambler: amblersurgical.com/7976e-kershner-reversible-lid-speculum-2-7-8-adult-size-thin-14-0mm-solid-blades-nasal-or-temporal-approach-adjustable-thumb-screw-tension
@@JJChopsticks thanks!
Did this patient underwent radial keratotomy before
Yes, you can see the radial scars on the cornea. The refractive targeting suggested by biometry is hard to rely on, so I personally would only suggest LAL for these patients.
@@JJChopsticks thank you also for the perfect site of corneal incisions I.e. between radial scars
Hello doctor. Why do you always have soft cataract patients? Can you show me a hard black cataract? Thank you
Great question :) I mostly do RLE on the West Coast of USA which means my patients are predominantly 50-65 years old and have "soft cataract" or as we call it, "clear lens". I have never seen a black cataract, unfortunately, I don't believe there are many currently in the USA.
Thank you for sharing your videos. I was wondering if you were using the MicroRec system to record your videos, or what system you were using? Thank you!
Hi, I use the Alcon Luxor Revalia microscope which has a built in panasonic video camera. I bring a USB drive which connects to the microscope and I record the videos with the microscope foot pedal controls.
😮
Where did his pupil go did his eye dilated in middle of surgery??
The pupil is darker in this patient, during nucleus disassembly, the cataract becomes opaque and the red reflex is dulled. The video camera then adjusts the white balance and further makes it harder to distinguish the pupil/iris from the cataract.
Hi Dr Tian, both my eyes are high myopic and both with moderate astigmatism however only the right eye has cataract. I plan to have the right eye done with monofocal toric. My ophthalmologist recommended me to do iol on the left good eye as well. He said I won’t be able to adjust if I have only one iol lens. What are your thoughts? Thank you
I completely agree with your ophthalmologist. When you have two highly myopic eyes and you only correct one with surgery, then you will end up with anisometropia (difference in prescription between the eyes) and aniseikonia (difference in size of images between the eyes). This is hard to adjust to beyond 2 diopters of difference. I would trust your ophthalmologist's recommendations.
@@JJChopsticks Thank you so much Dr Tian
Hi Dr Tian, after cataract surgery on one eye would I be able to return to work while I wait for surgery on the other eye? Or should I wait until both eyes are done before going back to work?
Hi, its fantastic technique I' m ophthalmology either from Taiwan, below was my video.
Welcome to my UA-cam channel :)
O que é isso no olho?
Hello sir. why did you do iol exchange? Does the patient have issues with panoptix iol such as disphotopsia?
Great question, yes, it is usually due to maladaptation with the technology (particularly Ringed lenses). Such patients may complain of rings around headlights while driving, which may cause anxiety and difficulty judging distance of nearby cars with nighttime driving. It is still a small percentage of patients who have this issue, most patients absolutely love this technology. You will see more IOL exchanges on my channel because I do them for other surgeons.
Nice Demo, l guess 25% of cases of perimum IOLs ended for lOL exchange ?!
I would say less than that, I do IOL exchanges for other surgeons so you see them more on this channel. If the reason for lens exchange is due to dysphotopsias/maladaptations due to multifocal lenses, then I would say 2-5% (estimation) result in an IOL exchange to a non-ringed lens and improves patient satisfaction.
Is the optiblue fda approved yet?
Yes, the J&J Symfony Optiblue has been out for 2 years now. Good lens!
I am rarely impressed watching other cataract surgeons. This is ridiculously impressive, Jack. Nice job, and thank you on behalf of all of us cataract surgeons struggling with back issues after years of surgery!
sir, is it better than Vivity or PanOptix?
They are used differently. One has rings and the other doesn't. The rings boost range but can be associated with negative side effects; the rings also limit the patient selection to very healthy eyes. The Vivity is shorter on range so a small offset can be introduced to boost the near potential. I would say patients favor and request the panoptix more often than the Vivity from my own experience, perhaps due to it being widely used in the USA. Both great lenses!

Heh just wondering what route number is this road?
Being we are going to be taking this road mid September it would be nice to see it normal speed!
