Ketogenic diets drop ESR and CRP very similar to GLP-1 agonist. What does insulin resistance do to Treg, NKT, B cell and macrophage function? NIH study showed that ketogenic diet increases NKT and Treg activity and decreases B cell activity. So while your waiting for GLP-1 agonists, cut all the carbs and target nutritional ketosis. I am 47 yrs old and recently started with a lot of head pressure along with scattered headaches, swallowing difficulties, etc.. I went to the ER and the CTscan showed: Scattered punctate parenchymal calcifications in the left frontal lobe, right periatrial region, and left parietal/occipital region. An additional calcification is seen along the right tentorium. this is the most terrible feeling I have ever experienced.
Eva I am 56 and two weeks ago my ct scan is also showing the calcifications in both frontal lobes. This is scarey shit! I hope that you can get to a university hospital for consultation and treatment. I wish you the best 🙏🏼

20 minutes to see an RA patient? Not in pvt practice with 22 patients a day to pay your bills. What is the NNT for stating? Compare that to our NNT for MTX, aTNF, or any other drug we use in RA. What about getting RA patients off of low dose Prednisone and pushing them back to primary care to more aggressively manage their BP whose drugs have a much better NNT?

Completely agree about spatial transcriptomics being the future, and hope it gains the traction it deserves. This could also be e applied to sjogrens and other tissue-specific autoimmune manifestations to inform the most appropriate treatments that might avoid global immunosuppression.
Excellent take! The newly retired group missed the ease of their paper charts. Some retired earlier because of EHRs. The EHRs drove them mad. Many werent allowed to learn to type in the old days, typing was only for girls.
Now do a placebo controlled study on HCQ for PMR. I cannot find one. I doubt it has any effect on tapering. Good to have data on MTX at 25mg. as docs are still pushing it, even as a steroid substitute! "She wants me off prednisone, so I'm supposed to switch to (MTX, Plaquenil, Fairy Dust)" Good luck with that.
I know a few people who have done brilliantly on HCQ, a few with MTX. But I am absolutely convinced that it comes down to it not being the same PMR they are talking about. PMR is the name for the symptoms - not the underlying cause of those symptoms. The greatest unmet need in PMR is a reliable diagostic tool. Without that - the rest is fluff and fairy dust
@@EileenHarrison-x9j Some people do brilliantly tapering off prednisone in under a year and having full remission too. We dont know who they are,. Men seem to get into remission quicker though. My 20 month journey has been constant flares, less than 1mg/month. My doctor somehow thought that 6 weeks on prednisone was scary and excessive, lol. It's often more like 6 years. Fired her and found someone with experience.
@@zootybeano After 16 years of work on a support forum for PMR/GCA and with research groups in the field, I can honestly say that in thousands of patients I haven't come across more than maybe half a dozen off pred permanently in under a year! And my own rheumy who is a world renowned expert reckons that they probably didn't have the PMR we mean!!! The guys on our forum get a bit cheesed off at "men do better" too.
What does "TDM" mean?
Off steroids in 6 months PMR? Even a year is inconceivable to me. Lots of PMR patients believe that MTX helped them taper. Any studies on HCQ? I can't find any for PMR.

Thank you for posting. In my experience, very fast and smooth to get diagnosis, hlab27 positive, crp elevated and mri positive for sacroilitis and peripheral arthritis. I hope they focus on a cure, I feel they have really nailed what to look for to diagnosis, great work over the last decade. Or focus on a treatment, not one drug has made it to market designed for axspa first. So many for RA and psoriatic, hope axspa gets a bit more attention, it seems 10 years behind RA and psoriatic.

Finally! A comprehensive, educated analysis of PMR symptoms that describe my experience of the condition. Thank you. There are so many ‘experts’ that stick to the shoulder, hip and knee pain ONLY, or it’s not PMR stance, which is both lazy of them and unhelpful for sufferers.

Afraid of sarilumab, after 2 years on prednisone, I think weakened tissues are causing diverticulitis, suddenly. Plus we can't see rheumatologists in our area anyway. Medicare won't pay for Kevzara unless administered in office. Either my PMR goes away or it doesn't. We added plaquenil and LDN but I don't see any real data on either one for PMR. MTX just shown to be worthless in a small study.
Real life, yes that matters. Gold standard is just nonsense. I'm on 3mg. after almost two years. I started on 20 for a month and absolutely had to split the dose. I had so many flares that I was able to taper by only 1mg a month and my CRP has done nothing but go up, latest 30. Sed rate always normal. I've never had "clinical remission." Maybe it will happen someday, or not. Reading thousands of forum entries by patients, Mayo Clinic, Facebook, quick remission and fast tapers seem to be rare exceptions. Seems men can get off faster.

Hard to understand and the transcript doesn't help much. So IL6 inhibitor (sarilumab?) gets frail patients off GC faster than methotrexate? In another abstract MTX was shown to be of no benefit. Medicare won't cover Kevzara unless the doctor administers it, is my understanding. No wonder the new rheum side gig is infusion centers!
The medication you keep mentioning, can you please spell it? Also, how does it compare to Embrel? I’ve been on Embrel for two years and it’s definitely helping but not 100%. I still have some joint pain in my hands and some swelling.
