Hello and welcome, I’m Fernando, a GP in the UK. Today we are going to do an up-to-date review of the diagnosis and initial assessment of COPD in adults according to the NICE guideline [NG115], always focusing on what is relevant in Primary Care only. I will be creating further episodes on the management of stable COPD and COPD exacerbations so stay tuned.
Right, so let’s jump into it.
The diagnosis of COPD is suspected on the basis of symptoms and signs and is confirmed by spirometry.
So, we should suspect COPD in people over 35 who have a risk factor (generally smoking or a history of smoking) and who present with 1 or more of the following symptoms:
· exertional SOB
· chronic cough
· regular sputum
· wheezing and
· frequent so-called winter 'bronchitis'
As part of our assessment, we will also check for signs such as:
· weight loss
· reduced exercise tolerance and fatigue
· waking at night with breathlessness and
· ankle swelling
Chest pain and haemoptysis, are uncommon symptoms in COPD and they should raise the possibility of alternative diagnoses.
The Medical Research Council or MRC dyspnoea scale should be used to grade the breathlessness, so let’s have a look at it:
· Grade 1 is when the patient is not troubled by breathlessness except on strenuous exercise
· Grade 2 is when the patient develops shortness of breath when hurrying or walking up a slight hill
· Grade 3 is when the patient walks slower than contemporaries on level ground because of breathlessness, or has to stop for breath when walking at own pace

Mine has been elevated for 30 years. Gama ggt normal Every test mentioned in this video was within reference. One test that was abnormal was bone fraction Very low. You appear to be extremely thorough and knowledgeable. Your patients are fortunate to have you. ☮️

Nicely summarised (no pun intended...) were there any changes to previous guidance?

No major changes from the previous guidance as such but they make references to the technology appraisals on SGLT2 inhibitors for heart failure which have changed relatively recently with dapaglifozin and empaglifozin being recommended for both HFrEF and HFpEF

Good day and thank you for the video. Would an Omeprazole prescription also cause elevated alkaline phosphatase ? My wife, 51 years old female always had normal levels along with everything else. Earlier this year she was prescribed Omeprazole and now has triple AlkPhos levels. She's awaiting further testing.
Thank you for your comment. Omeprazole is not typically associated with significantly elevated alkaline phosphatase levels. However, as with any medication, individual responses can vary and there have been rare reports of transient liver enzyme changes in some people taking omeprazole, meaning that some people may be more sensitive to medication effects. I am pleased that she is having further investigations, so that you can all put your mind at rest. There may be a simple explanation for this abnormality 🙏

Thank you - this was so informative! On my recent test my ALP went from 47 (last year) to 58. I am a 50 year old woman. Should I be concerned? Could excess exercise cause a temporary increase?
Thank you for your comment. Your readings are not concerning in principle but you should discuss all your results and your overall health with your doctor. They are in the best position to advise you 🙏
At 5:06, why isnt it stop amlodipine and switch to acei, whkch would be step 1. Why still keep offending med and try step 2 drug
Thank you for your comment. I never looked at it the way that you have interpreted it and I now think that the wording that NICE has used is confusing. What they actually mean is that, if the CCB is not tolerated, then you stop it and substitute it with a thiazide-like diuretic. So you are giving the thiazide-like diuretic for their hypertension, not for the oedema. The reason why you would favour a thiazide-like diuretic over an ACEI or ARB is because in this group (age >55 and Afro Caribbean people), thiazide-like diuretics are more effective than ACEIs and ARBs. I hope that his makes sense 🙏

I have had this for over 5 years. No one has been able to tell me why but I do have a vit d deficiency and Hashimoto’s thyroiditis.
Thank you for your comment. Hashimoto’s thyroiditis is an autoimmune disease and the true cause is not clearly understood. It could be genetic, environmental or a combination of the two. Vitamin D deficiency should not be longstanding as long as you have adequate initial loading supplementation to correct it and long term maintenance thereafter. I’d recommend discussing this with your doctor again to make sure that you get all your questions answered 🙏

Thank you for your comment. No, daridorexant is a type of drug known as a dual orexin receptor antagonist (DORA) which acts by blocking the activity of orexin. The orexin system promotes wakefulness so by blocking it, daridorexant helps sleep

Thank you for your comment. Submucosal fibroids can give you hassle with bleeding and I would simply recommend that you consult with your gynaecologist. A lot of the advice is down to the details of your case 🙏

Thank you sir. For the past 4 yrs i have been having higher bp in clinic setting but optimum bp at home between 99 to 107 sys. And 63 to 73 dias. Doctors seem not to believe me when i told them i only have high bp at the hospital. I refused to take the medicines. I recently did my blood work. Everything was normal except pericardial fluid at right ventricle measuring 0.85cm. i am not diabetic, good BMI, i try to live a healthy life, i exercise, kidney and liver function test were okay, triglycerides normal, ldl a little bit high 4.96 mmol but i notice my hdl is low 0.92.
Thank you for your comment. I’m pleased that your BP at home is good. Any specific concerns should be discussed with your doctor, as they have your full medical records but by what you describe, it doesn’t seem that you have anything to worry about 🙏

Thank you for your comment. I really appreciate it. NICE are working with BTS/SIGN to produce UK-wide guidance that will update and replace the current NICE guideline. The methodology is also different. BTS/SIGN use literature appraisal and clinical studies whereas NICE combine critical appraisal with health economic modelling. The differences in the guidance are probably varied. I have not compared them both in detail but this article, although from 2017, will give you a flavour.
