    Normally when we begin these things it is with an understanding of the root problem. This is offering solutions but what is the root problem? And when we speak "outcomes" - is the number of knee replacements the outcome? Sounds like just trying to pump a bunch of interventions out of the Maple Leaf factory. Activity Based Costing has been done in industrial processes since about forever. But to do so you need to design a process - maybe that's the root problem in hospitals? Their processes are too cumbersome, cost too much, the quality isn't there, who knows? I don't like this piece because I am being fed something pretty specific by individuals who may not have my interests at heart. A VC running the show makes me wary. What are the outcomes we seek, and how do we measure these?

    Activity-based funding - I have an honest question: - This incentivizes volume, but not quality. How do you mitigate unnecessary surgeries being performed to boost budget? How about quality? Wouldn't it be to the hospital's advantage to have surgical complications that require more surgeries? While I agree that block funding is ineffective and results in top heavy healthcare administration that does little to benefit patients, I am curious about what measures are in place to prevent misuse of activity-based funding.

    OK! So these people are going to a PRIVATE hospital, it's clearly marked, how does that work? The government is paying for us to get a surgery in a private hospital it sounds suspicious 🤔, or do we have to have a private health insurance? Thanks for responding to me, cause my husband was on a waiting list for 21 years in here Australia 🇦🇺, now I'm taking him overseas

      Hi Fiona, the patients pay for surgery, then are reimbursed by the Irish government. I'm unsure whether Australia has a similar program, but it would likely be worth looking into! -Dom

    If they had the money to afford reimbursing the health care costs of the people who went to the US for treatment in the first place, we wouldn’t be having these long waiting lines. The main issue with health care in Canada is funding. They cut so many people and ressources off to be able to afford it (which they still don’t, they are in huge deficit). Doctors aren’t staying because the pay is low, and those who do are facing a huge influx of patients their colleagues who quit left them with. They recently told us to brace ourselves with the increased amount of patients due to aging population, WHILE doing a staff cut because we are billions over budget. Maybe this would have been a solution if Canada was located in Europe. But with the US where the cost of even the most basic of treatment is through the roof? lol.

    My brother-in-law had an agressive form of prostate cancer & it couldn't be removed. He had to wait over three months for radiation. Who knows how far it spread during that wait time.

    This is completely wrong. Land is a perfectly inelastic supply, so the tax incidence on a real estate transactions falls almost entirely on the seller. Removing or reducing this tax will just result in home seller getting to keep more of the money. It will have almost no effect on the price tag.

    I paid 125k in tax on building my house 5 yrs ago. a 3 bedroom 2600sqft, not fancy.. and the tax rebates... no gst rebate on houses over 400k.. not even prorated like pst.. just theft plain and simple.

    All care improvement options seemed reasonable. The government price paid for any procedure would need to be the same amount that a doctor was paid no matter where the procedure was done. If prices paid varied, all doctors would move to wherever they made the most money. That would increase wait times at all facilities because lower paying facilities would have fewer doctors and higher paying facilities would have all the doctors and their hospitals would become oversubscribed causing line-ups. This wouldn't help wait times and would still raise prices. People who could pay privately would have more choice. So the system would benefit the wealthy. Sounds like our Southern neighbors. Their system is horribly broken.

    This sounds great, but will provincial governments or the federal government be willing to pay for this? Except in a few cases, no. The reason that there are waiting lists is because health care is rationed and health care is rationed because there is a limit on how much our governments are willing to pay for healthcare. The additional money that would be needed to fund these medical procedures will not fall from the sky.

      The ugly secret is the Health administration consciously knows damn well they *could* contract it out for far less than their bureaucracy-mired departments can provide it, and on a incomparably better timeline. Maybe the simplicity of making it someone else's problem for a mere fee - rather than a 5-7 year fight to "plan" and another 5y to "construct and outfit" will attract more support of the case workers and low level admins and the political side can reach across to them? The problem is the middle and upper administration who make careers out of that dragged on procrastination. It's literally criminal in hundreds of applications. Possibly the political parties making it a political football, too. Here in Edmonton they appear to plan to run 1/3 of the city without reasonable access to a hospital, with the stoppage of the Ellerslie Road hospital. That *could* have been less than a year away from use if the plan had been followed. The "PPP tower clinic" that was supposed to replace Ericksen Nissan is apparently shelved, too. Punishing Edmonton for how they vote? Part of me fervently hopes so, despite living here. Such idiots, such evil. Look at the stupidity of closing schools in the Edmonton core just as waves of immigrants to those areas start having medium to large families. There are daycares on literally every block, but do you think the school board can maintain schools in those areas? Nope, one after another being closed. And bulldozed and turned to.. parks!? The difference, the daycares are private and un-unionized... the school board union is a literal monster and the government does its share to prevent "school service" from being offered. Pretty much as perfect a case study as the North and South Korea lights-at-night meme.

      2nd Street has another excellent but longer video on how healthcare services could be improved. Right now (at least in BC where more than 1M people don't have a Dr.) funding here is based on Bulk (may be the wrong term) funding so the hospitals receive a specific amount of $'s for the year but other western countries with a mix of private and publicly funded hospitals operate on an activities based funding (ie.the number of surgeries etc. that are performed). Canada is apparently the only country with exclusive universal healthcare (no private) other than N. Korea. All of the other countries have a combo of universal and private healthcare without it costing more $'s because they operate more efficiently. Plus people get the care they need when they need it because there aren't excessive wait lists which can ultimately end up costing more tax $'s because people can end up on disability, welfare etc. which also has costs. No other country (at least western countries) look at Canada as an example of the type of healthcare system they want to implement....just my opinion! Oh yes and of course politicians, judges, federal prisoners and more, are exempt from having to be on a wait list, they get their healthcare when they need it!

      My wife was put on a waiting list for hip surgery after being on physio therapy for a year that Alberta health care wouldn't pay for . So rather than waiting another year or so in deep pain , we decided to procure the procedure in a private clinic . We live in Edmonton so we were forced to go to Toronto even though there was a private clinic in Calgary. This cost us an extra 6k , in hotels , airfair and meals , total bill 34,000.00 which Alberta health care reimbursed us 1,300.00 dollars. Thank God we could afford this to end my wife's suffering.

      @@brianmorton2171 I believe most people in Canada think we have universal healthcare (ie. 1 tier system) when the reality is that we actually already have a 2 tier system that basically operates on whether or not you have "money" or you don't. If you have the $'s, you can afford to get the healthcare you need outside of our basic healthcare system but even that part is convoluted. If you live in Quebec, you can get private healthcare that you pay for within the province (they have some legal/Gov. provision that allows for that) so unlike you, who had to pay all those extra expenses to get the medical help you needed, you had to go out of province which I believe is true for every province except Quebec. I think this is absolutely wrong and my perception is many Canadians aren't even aware of this discrepancy. I'm glad to hear that you did get the healthcare needed for your wife, albeit at such a financial cost to yourselves and I just hope that if I was ever in a similar situation I would have the $'s to pay for it as well. However, in the interim, our tax dollar's, which are not insignificant, are being paid into a system that is seriously flawed....just my opinion!

      Right. So just as there is monetary arbitrage, there is labour arbitrage (the deindustrialization of the West). This is medical services arbitrage, and needs to work on that basis. You have to realize Ireland has the EU's highest standard of living (some competition from Switzerland is now eclipsed, Monaco and one of the Benelux countries may still exceed them). Therefore, any country they are going to has a lower cost. That's kind of key to this working. You hear it about tourist dentistry to Mexico. Also cosmetic surgery, though I don't "run in those circles" myself. So, Mexico is very much an option. US just isn't, that I can see. Large areas of Europe would be possible, maybe more so for Canadians with connections to there, or if a business plan was made. At one point about 25years ago, Italy had the highest number of doctors per capita in the world. However an international flight is a very far cry in cost, from a day bus. A decade or two ago Cuba was being presented as a possibility, but I tend to wholly believe reports it is a country in collapse. I believe there is some province to province swapping between Alberta and Saskatchewan, but don't know the details.

      two more thoughts: Not sure if links are allowed in YT comments, so I'll cut and paste. The facility they are going to is: "Kingsbridge Private Hospital [...] a private hospital on the Lisburn Road, Belfast, Northern Ireland. It is owned by Kingsbridge Healthcare Group, the largest provider of private healthcare in Northern Ireland." So the big difference that SecondStreet regard as a "3rd rail" to explicitly discuss, is that the governments have to get over the fear of "privatization" that has been propagandized into a cross section of Canadian citizens- that section I will call out as a mix of idiots and pure evil, and I treat them as such. So the question is, why not allow the money to stay in Canada, by building private facilities, employing Canadian workers right from those who lay the concrete foundations during construction, to all the staff, to, well... gosh I'm going to blurt it out: the *owners* of the hospital. I can tell you why not: because the far left idiots would rather cut off their nose to spite their face and not admit their failed life paradigm, and to thus drag everyone down with them, spitefully and full of hate for those who see the truth. The other thought is North Dakota and Montana are mentioned. The US system, every bit as flawed if not moreso, is closing down rural hospitals at a huge rate. It's consolidation of the industry- driven by legalities, insurance group coverage, population stagnation or loss in the so called "flyover states" (and Illinois), and so many other things I may not be informed in detail of. Perhaps there *is* a struggling regional health company there that we Canadians could throw a lifeline to? That would be win/win in that local folks could as a side benefit retain *their* access to local hospitals due to the influx of new revenue. Not near as good as building our own, but as long as we keep retaining the "cut the nose off to spite the face" clique as a dominant voting block, it is the best step forward. Which provincial government will take the step?

      I don't have enough time to watch all of this, but I bet this video is going to hint at merely travelling from one province to the next. Yes, that won't work, but it does help the provincial premiers pretend that the Canada Health Act (and its portability clauses) don't exist. Second Street always pushes privatization at some point
