a good example that we are working on it sharing the multi factorial falls assessment across inpatient and community team so falls risk is identified and intervention to prevent falls is started and shared which reduces duplication ... SystmOne is a great vehicle for reducing duplication

Thanks great example with Physio first and physiotherapists are leading the way in primary care but I am sure there are other ahp professions that could provide input to support primary care demand. I think early identification of frailty is key , Pre diabetes , return to work are ideas The demand at ED departments could be reduced by falls response type service with our paramedic colleagues . There are plenty of examples that are reducing conveyance and admission to hospital . For the future we need to explore advanced roles such as physio to support staffing and extend skills in ED minors

It is important that we think about health and wellbeing of our local populations. I think AHP's should be doing more preventative interventions. I think AHP's are great at reinforcing Public health messages but we should be delivering more in where our impact will be greatest . Frailty and falls are a key area. We should be actively identifying people at risk of frailty in a more preventative approach. This active case finding would be best undertaken in a primary care setting
