After reading you comment on how you believe it was Kiara, I think you solved it. The only other person I could see doing it is the Goombus. She gets on to solo explore, digs a hole down randomly, dies while singing "don't dig straight down", and ADDs off to do something else.

the hole was caused by someone who was afk. common glitch causes person to still mine while in the pause menu. they would've never realized it happened and just came back to a death screen
