    I’m an old guy … Retired Chartered Accountant with an MBA…gramps…qualified PPL last year…I’m mentoring students alongside their using Bristol Ground School. Both my Daughter and Son are industry professionals. I have worked alongside female professionals and hired females…I’m appalled by the personal questions you were asked. I bust my sides when I hear old school guys assuming 4 ringers are stewardesses…flight attendants. I cannot learn enough about females being honestly regarded as equals in any job they choose. My daughter has been underpaid for decades by a huge MNE because she is female. She has just changed job and appears to be regarded as at least equal to males. She has a beautiful 15 yr old daughter who is way more intelligent than me. Thanks so much for this talk.

    Such a powerful presentation about this horrible disaster, Captain Sorvino. You told it both personally & professionally. I would think that your colleague, Bob, suffered tremendous PTSD as the result of his experience and survival. Sending that many aircraft to the limited Tenerife destination from the original speaks poorly of the decision making capabilities of the party making it. Looking forward to further YT presentations from you.

    I watched your video. I’m a private pilot ASEL and a mechanic, b757 mx instructor, trouble shooter and ended up in maint control for a major. I took a lot of charters when a mech was required and did a lot of road trips to recover busted airplanes. I have a bachelor’s and master’s degrees. My flights were pre CRM. I was nearly killed four times on three flights. Asshole captain attempted to kill us on takeoff and landing. A controller at Tunis Tunisia anf anand an Air France crew gave us a super big scare by closing with us in the dark with their lights, including nav lights, turned off. The only guy that tried to stop the KLM captain was a fearless PFE. The only I guy I witnessed correcting a captain was a PFE. We were in a barberpoll descent and the cabin altitude was going to hell. The PFE pushed 2 and 3 up to spin up the TCs to get more in the cabin. When the captain attempted to pull the throttles back the PFE push them back up and told the captain to pull the nose up a little. The klm captain was responsible to ensure the runway was clear regardless of anything. I know old guys who were thrown out of the FE seat in the 1962 strike so a pilot could have the job.

    Wow, Thanks so much Laura- BY FAR the best explanation of this horrible accident that I’ve ever heard. So many links in the chain, so many holes in the Swiss cheese. So damn Sad. (My Qantas career was actually very similar to yours, and I’m now happily retired. My favourite aeroplane of all time was the 747-400.)

    Apparently the man who pushed the throttle wide ope😮n caused the crash. Why did he push the throttles is the problem. Everything else is a side show.

    Wow… I was stationed at Spangdahlem AB in Germany when this happened. I have read a ton of articles and seen just about every documentary that’s been made about it, but never been as affected as I was by this. Great video! P. S. I just ordered your book!

    That was an awesome webinar and very thorugh. I have studied this accident very closely but you brought up a couple of things I was not aware of. You are absolutely correct that there were numerous times the accident chain could have been broken but I want to zero in on the last error. First let me say that I am not putting the Captain down, he was under considerable pressure do to duty regs and the serious ramifications of exceeding them. In fact I would add the Dutch laws on duty time as a contributing factor. The last chance to avert the disaster past when the Captain began the takeoff without a takeoff clearance. They received a route clearance but for a takeoff clearance to be valid the controller must say route clearance but for a takeoff clearance to be valid the controller must say “Cleared for takeoff” and they did not. That does not put all the blame on the Captain though. This accident is one of the reasons I stressed proper phraseology when I was instructing. I am certainly not saying that if I were in that left seat I would not have done the same thing. It is easy to lay the blame completely on him but we are all looking at it with 20/20 hindsight. The best thing we can do is learn from the mistakes of others so that we do not repeat the same mistake. Your webinar will go a long way to accomplish that goal. Once again, very good job with this accident study. I also learned CRM while with the airlines and still use it today when driving with my wife. At an intersection her job is to tell me when the traffic is clear from the right and when she tells me it’s clear and the left is clear we enter the intersection.

      The requirement for ATC to state, "Cleared for Takeoff" came from this accident. (keep in mind - this did not occur in the United States) In 1977, the word 'takeoff' could be used at any time by controllers globally. Mandating phraseology, along with mandating the requirement to speak intelligible English while using that new mandated phraseology - was all new 'after' this accident occurred. Now if only they could mandate a sterile tower cab environment, as airline crews are required to practice.

    Every other account I've heard of this horrible incident has focused on the KLM pilot is being the bad guy. I am so glad I took the time to watch this presentation, because it once again (as truth always does) shakes out all the unanswered questions and missing data that the 'official narratives' seem to be known for. Quite an eye opener to say the least. Those people who were lost are owed an honest account of their demise. Finally, they have it.

    Congratulations to this very precise analysis of this accident. However, back in the days I read in aviation magazines that according to the transcript of the cockpit voice recorder of KLM, the copilot supposedly told the captain, that actually had not received a take off clearance. According to that information it seems that at least there was some uncertainty about whether or not a take off clearance had been issued. This of course would, if true, put the ultimate blame on KLM.

      Not accurate information regarding the first officer - which is exactly the point of this presentation. Read the cockpit transcripts, rather than magazines.
