North Western Melbourne Primary Health Network (NWMPHN) is part of a nation-wide group of Primary Health Networks (PHNs).

Each PHN aims to increase the efficiency and effectiveness of health services for patients - particularly those at risk of poor health outcomes - and to improve coordination of care to ensure patients receive the right care in the right place, at the right time.

We acknowledge the peoples of the Kulin nation as the Traditional Owners of the land on which our work in the community takes place. We pay our respects to their Elders past and present.
