Apremilast wont make it last
I was diagnosed with PMR in the early 2000's. It took me three years of chronic debilitating pain to get the correct diagnosis, because I was barely into my forties at the time, and doctors never even considered PMR because they believed me to be too young to have it. Even though all my symptoms were clearly pointing that way. I was given prednisolone and the pain disappeared almost over night, as though it never existed. I was so ticked off because if the multiple doctors and specialists I saw, could've just opened their minds to think outside the box a bit, it would have saved me three years of abject misery. More doctors need to think of zebras instead of horses.

Thank you for your information. However, I have a question. The refeeding syndrome may lead to metabolic acidosis. Why? Actually, when there is hypokalemia, doesn't it lead to metabolic alkalosis ?!!!

i have this white frothy substance in my mouth every morning and i noticed it like about 4 to 6 months ago, my doctor is having me tested for Sjogren, but i only have that symptom, i do have dry eyes but not all the time

I didn't get the idea why we used thiamine...?
Patients presenting with alcohol related portal hypertension and oesophageal varices as a complication would be high risk for B1 deficiency. It's something easy to forget to give and this population may be at risk. In some ways, I guess its slightly presumptuous but I like to have it in my memory as a reflexive prompt - is this patient likely to be at risk of wernicke-korsakoff syndrome?
The answer is YES. Wernicke-Korsakoff syndrome is commonly associated with "thiamine (vitamin B1) deficiency," which is prevalent in patients with chronic alcohol use. These patients are at risk because alcohol impairs thiamine absorption and utilization. Given that the patient mentioned that he is presenting with alcohol-related complications like portal hypertension and oesophagal varices, they are indeed likely to be at risk for Wernicke-Korsakoff syndrome due to their higher likelihood of thiamine deficiency...

saag score cut off is 1.1 not 11
Good observation - however, this really depends on what units you use! The SAAG score of 11g/L (which is the units I used in this video and is common in the UK and Australia) is equivalent to 1.1g/dL (which I think is commonly used in the US).
