Hunter in Gadgetzan died because he triggered the attack ability on his pet. The pet attack was bound to the SWU (shift-wheel up), button lit up so he had move the mouse wheel. Guards were attack-able because the player was still neutral with Gadgetzan faction (we know this because the guards have yellow name plates). Goblin bruisers don't mess around they attack instantly when ability is triggered they don't wait for the attack to land.
